Access to transportation is a Social Determinant of Health, yet many rural Vermonters face barriers to adequate transportation. Consequently, patients face difficulties accessing primary care and completing recommended health screenings. Destigmatizing ride programs and effective communication are key components of connecting patients with transportation resources. This project sought to develop a reference tool for clinic staff to improve communication about transportation barriers with patients and ultimately improve health outcomes.

Inconsistent access to reliable transportation is a significant barrier to medical care in rural New York. Lack of transportation results in missed medical appointments, poor management of chronic conditions, and increased preventable emergency room visits. Increasing routine screening for this social determinant of health can reveal underlying barriers to care, promote referrals to case workers and increase resource allocation. This project sought to increase routine screening via the promotion of smart phrases to be included in routine wellness documentation in the EMR, in conjunction with a didactic session for current family medicine residents.

COVID-19 continues to be a significant health threat and has caused significant disease and mortality in the fall 2021 wave largely driven by the Delta Variant and complicated by unvaccinated patients. In the state of NY, approximately 40 patients died per day (7 day moving average) from serious COVID-19 illness during this time period. During this time, approximately 35% of Clinton County’s population is not vaccinated for COVID-19 and is subsequently unprotected from serious COVID-19 illness. The survey was dispersed to all medical students at LCOM as well as third and fourth year medical students at Ross University and American University of the Caribbean (AUC). Acute antibiotic courses are known to cause gastrointestinal side effects including nausea, bloating, and diarrhea. Patients at Newtown Primary Care in Newtown, CT being treated with antibiotics for upper and lower respiratory infection often present with these side effects. These symptoms may deter patients from taking the full course and promote bacterial resistance, delayed recovery, and increased patient healthcare costs. Safely tolerating antibiotic regimens is important especially in those at high risk for Clostridium difficile colitis. In fact, it is the 7th most common reportable infectious disease in US with an estimated over 300,000 people diagnosed and treated each year. The total medical cost attributable to Lyme disease has been estimated to be as much as $1.3 billion a year with the total cost of Lyme disease testing estimated to be $492 million. Marijuana is the most commonly used illicit substance for pregnant women nationwide, with rates of use trending upward. Marijuana use in Penobscot County is elevated well above the national average, and is likely to continue to rise with the legalization of recreational marijuana in the state of Maine. Though the American Congress of Obstetricians and Gynecologists (ACOG) discourages marijuana use during pregnancy and breastfeeding, studies have shown that providers do not emphasize counseling pregnant patients on marijuana use. Human Papillomavirus (HPV) is currently the most prevalent sexually transmitted infection in the United States with an estimated 79 million people infected. In the state of Connecticut, HPV vaccination is not required for entrance into the 9th grade and because of this, there is less awareness of the importance of HPV prevention and immunization among high school students. As of 2013, 37.7-49.9% of females ages 13-17 living in Connecticut completed the HPV vaccine regimen. My objective for this project was to promote awareness about HPV infection and HPV vaccination among high school students.
