Background: Antiretroviral directly observed therapy, in which nurses or other allied health professionals provide patients with daily medication, is an evidence-based solution for viral load suppression in people living with HIV who experience homelessness. Purpose: This quality improvement project aimed to assess the outcomes of antiretroviral directly observed therapy at one urban clinic caring for people living with human immunodeficiency virus who experience homelessness and substance use disorder. Methods: Data was collected from the electronic health record for the antiretroviral directly observed therapy patient cohort (n = 33); 10 of them were surveyed. Healthcare policies are increasingly calling for diversification of the workforce to meet healthcare needs. Community health workers (CHWs) are understood from the literature to be effective at bridging the gap between patient and healthcare provider, especially for vulnerable populations. Substance use disorder is a significant problem in the United States causing considerable distress and cost to the healthcare system as well as familial, community, and societal resources. Based on evidence-based research, a statement by the United States Preventive Services Task Force in 2015, recommends substance use disorder screening for adults in the primary care setting. A review of the literature concerning screening tools for substance use disorder resulted in twelve studies being included. Background: The number of newborns exposed to opiates quadrupled in the United States from 1999 to 2014, from 1.5 per 1,000 hospital deliveries to 6.5 per 1,000. Many of these babies are admitted to the neonatal intensive care unit for neonatal abstinence withdrawal syndrome. While neonatal nurses are trained to care for babies exposed to substances in utero, they often struggle to meet the unique psychosocial needs of the babies’ parents. Problem: Individuals with substance use disorder (SUD) have disproportionately higher rates of unintended pregnancy when compared to the general population, estimated to be 85% (Heil et al., 2011). Not only are poor maternal and fetal outcomes associated with unplanned pregnancies, but pregnancies in women with SUD are further complicated by additional risks and adverse outcomes (Black & Day, 2016).
