Case Report Papers

Background and Purpose: Knee osteoarthritis is the most common joint disorder in the elderly. The prevalence of unicompartmental knee arthroplasties (UKAs) increases by 30% each year. Benefits of UKA’s are quicker recovery times and an overall less invasive procedure compared to a total knee arthroplasty (TKA). Robotic-arm-assist surgery has been shown to increase the accuracy of implant positioning compared to traditional surgical techniques. The purpose of this case report was to look at the impact of physical therapy (PT) on outcomes and return to golf in a patient following a Mako robotic-arm-assisted UKA. Case Description: The patient was a 71-year-old …

Case Report Papers

Background and Purpose: Plantar fasciitis (PF) is present in 2 million Americans a year, and in 10% of the population over a lifetime. Conventional physical therapy (PT) for PF has included strengthening of foot and hip musculature, and lower extremity stretching. Extracorporeal shockwave therapy (ECSW) has also been utilized with success in treatment of PF. While these two treatments have been used individually, their combined effect has not been studied. The purpose of this case report investigated a combined plan of care utilizing conventional PT, targeted hip strengthening, and ECSW. Case Description: The patient was a 48-year-old female who presented …

Inpatient Physical Therapy Management For A Patient With Chronic Pulmonary Complications Secondary To Multiple Lobectomies: A Case Report, Bayley Archinal

Case Report Papers

Background and Purpose: Bronchiectasis is a disease defined by abnormal dilation of the bronchi from recurrent infections and/or chronic inflammation, which can lead to irreversible lung damage. Based upon severity and patient response to conservative treatment, a lobectomy may be required to resolve their symptoms. Though surgery may provide symptom relief and improve overall quality of life, patients are likely to have long-term pulmonary complications. There are several studies regarding the treatment of patients with pulmonary limitations secondary to a lobectomy. However, there is little to no evidence regarding the long-term treatment of patients who have undergone multiple lobectomies. The …

Case Report Papers

Background and Purpose: People with Charcot-Marie-Tooth (CMT) disease have a greater risk of falls and subsequent fractures. There is a scarcity of information regarding the most effective rehab protocol for a patient with lower extremity fractures and CMT disease. Utilization of the AlterG® has been shown to promote early mobility in patients with an orthopedic diagnosis, which is also vital for CMT patients, but has not been fully investigated. The purpose of this case report was to investigate a comprehensive physical therapy program, including use of the AlterG® treadmill, for a patient with lower extremity fractures and CMT disease. Case …

Multimodal Physical Therapy Management Of A Patient With Unilateral Neglect Post-Stroke In An Outpatient Setting: A Case Report, Meghan Riley

Case Report Papers

Background and Purpose: Unilateral neglect (ULN) syndrome negatively affects recovery post-stroke and is often associated with falls, longer stays in rehabilitation, and the need for more assistance at discharge. Typical physical therapy treatments include voluntary trunk rotation, visual scanning exercises, mirror therapy, hemispheric activation, sensory awareness training, and lower extremity loading but the efficacy of these treatments is still unclear. The purpose of this case report is to describe multimodal physical therapy (PT) management that includes common PT interventions to alleviate symptoms of ULN and report upon the outcomes of the various treatment methods for rehabilitation of neglect during functional …

Background/Purpose: The McKenzie Method of mechanical diagnosis and therapy (MDT) is supported in the literature as a valid and reliable approach to spine injuries. It can also be applied to the peripheral joints, but has not been explored through research to the same extent. This method sub-classifies an injury based on tissue response to mechanical loading and repeated motion testing with repeated motions identified during testing used to guide treatment. The purpose of this report is to demonstrate the assessment, intervention, and clinical outcomes of a patient classified as having a shoulder derangement using MDT methodology. Case Description: The patient …

Case Report Papers

Background and Purpose: Low back pain and lumbar hyper-mobility are common during and after pregnancy. Postpartum low back laxity can contribute to LBP and can become chronic if not addressed. Core stabilization exercises (CSE) have been shown to improve function and reduce pain in patients with nonspecific chronic low back pain due to lumbar instability. Additionally, Medical Exercise Therapy (MET) has shown good outcomes in reducing pain in patients with LBP but has not been thoroughly investigated in the treatment of chronic post partum LBP. There is limited research reporting the use of a combined treatment protocol utilizing CSE and …

Background and Purpose: Stroke is the leading cause of serious long-term disability for American adults. Most stroke survivors receive physical therapy (PT), and task-oriented rehabilitation is one novel approach known to benefit stroke survivors. The purpose of this case report is to illustrate the outcomes of a task-oriented approach to PT interventions on a patient >12 months post stroke. The unique aims were to 1) outline possible benefits in function from repetitive task-oriented training techniques and 2) document outcomes of a patient who had received PT services >12 months post stroke. Case Description: The patient was an 82 year-old female …

Physical Therapy Management Of A Patient After A Subacromial Decompression With Acromioplasty And Bursectomy: A Case Report, Brianne Landholt Dec 2014

Physical Therapy Management Of A Patient After A Subacromial Decompression With Acromioplasty And Bursectomy: A Case Report, Brianne Landholt

Case Report Papers

Background: Subacromial impingement results from repetitive trauma to structures underneath the subacromial arch leading to a decrease in the subacromial space, impingement of soft tissue, and ultimately a decrease in functional abilities. Conservative treatment includes physical therapy, nonsteroidal anti-inflammatory drugs, and corticosteriod injections. If improvements are not observed, there is a surgical option of a subacromial decompression (SAD). Conclusive evidence supporting surgery over conservative measures does not exist1. Literature is lacking regarding SAD without rotator cuff (RTC) repair and performing SAD with both an acromioplasty and bursectomy. Therefore, the purpose of this case report was to investigate the functional outcomes …

Although there is evidence to support a high success rate to return to work in the general population and to sports play in athletes following discectomy, less is known regarding the interventions that played a role in their return. The following case describes the interventions and management of a recreational football player following discectomy. Case description: The patient was a self-described highly active 24-year-old male post discectomy. The patient had pain, poor body mechanics, gait abnormalities, range of motion deficits, and strength deficits of his low back and lower extremities. Physical therapy interventions included soft tissue mobilization, stretching, exercise, and …
