BACKGROUND:

The U.S. Hispanic population has the highest rate of Type 2 diabetes mellitus (T2DM), which is influenced by socio-economic status and language barriers. Current evidence suggests that the delivery of culturally-directed Diabetes Self-Management Education (DSME) with the use of an interpreter can reduce diabetes-associated complications.

Background: Clostridium difficile infection is one of the most common healthcare-associated infections. Careful screening upon hospital admission enables clinicians to mitigate the spread and sequelae of this illness through early intervention and isolation but this screening must be accurate and highly sensitive to maximize benefit.

Local problem: The site for this project, an ICU in Tennessee, currently employs a highly sensitive but nonspecific tool to detect CDI. This has resulted in low accuracy, low staff compliance, low patient morale, and unnecessarily high personal protective equipment (PPE) costs.

LOCAL PROBLEM: Setting included 15 retail health clinics operated by Vanderbilt University Medical Center in Middle Tennessee. Project examined provider education, patient engagement, and commitment to antibiotic stewardship, components of the CDC's Core Elements of Outpatient Antibiotic Stewardship framework. BACKGROUND: Heart valve disease prevalence is expected to rapidly increase over the next twenty years. Aortic stenosis, the most common valvular disorder, is a significant cause of heart failure hospital admissions and high mortality if left untreated. Treatment options include surgical aortic valve replacement (SAVR) and the more popular transcatheter aortic valve replacement (TAVR). BACKGROUND: Colorectal cancer (CRC) is the third leading cause of cancer deaths among men and women in the U.S. CRC is preventable and manageable when detected early. Approximately 30% of average-risk Americans are overdue for colonoscopy screening. Colonoscopy screenings can reduce death by 67%. Current evidence suggests CRC screening education improves screening uptake among men and women at average-risk age.

Background: Overdose deaths have exceeded US war fatalities since WWII. Central Appalachia faces high substance use rates and poor health determinants. The opioid epidemic costs $740 billion annually. Providing 100 million chronic pain sufferers alternative pain control is efficacious, equitable, and reduces opioids. The author collaborated with a nurse-led mobile clinic in Central Appalachia to identify evidence-based non-opioid pain relief options for practice translation. In patients with hypovitaminosis D, the body will increase parathyroid hormone production to absorb calcium resulting in skeletal or muscular pain. Opioid pain medications are currently viewed as a standard part of pain control in the vast majority of surgical procedures. However, opioid pain medications can have harmful effects on patients leading to increased length of hospital stay, unplanned readmission, and contribution to relapse or development of opioid use disorder. An opportunity for improved practice of pain management lies within opioid alternative pain medications such as dexmedetomidine. Ventilator-associated pneumonia (VAP) is a hospital-acquired infection that affects intubated patients on a ventilator. The risk of acquiring the infection increases the longer a patient stays on the ventilator. Complications from ventilator-associated pneumonia can impact the patient physically, financially, or emotionally. Evidence-based recommendations collectively implemented to prevent VAP are referred to as a care bundle. Although research evidence supports religiosity’s predominantly positive mental and physical health benefits to patients coping with varied health problems, there are few studies exploring the influence of religiosity on coping with sport injuries among athletes identifying with specific religions. This study examined the relationships between religiosity and the use of religious and non-religious ways of coping with sport injuries by athletes affiliated with diverse Christian denominations. Traditional ovariectomy techniques for heifers (e.g. colpotomy) have adverse effects ranging from hemorrhage to mortality. Electrosurgical bipolar vessel sealing devices offer a superior ligation of ovarian pedicle and associated structures. In ruminants, standing flank laparotomy has been used for abdominal visibility. We performed ovariectomy in 2 companion beef heifers via a unilateral flank approach, combined with electrosurgical ligation. Surgeries were performed in standing animals, under local anesthesia, with no observed short- or long-term adverse effects. This technique is worth considering for cases where the security of electrosurgical ligation combined with the accessibility of a flank laparotomy are desired. Additional research …

The purpose of this study was to collect and tabulate data from case studies about ventriculoperitoneal (VP) shunt malfunctions and determine if correlations exist between the possible cerebrospinal fluid (CSF) shunt malfunction causes and the symptoms of these malfunctions. Close associations between the two variables would contribute to identifying causal relationships, to decrease time between onset of symptoms and diagnosis to avoid invasive procedures during the diagnosing process.
