Bolstering Moral Courage And Ethical Decision-Making Confidence To Moderate Nurse Leader Moral Distress, Lisa Krening

Central Division Nurse Clinical Inquiry Conference

Abstract:

Bolstering Moral Distress Knowledge, Ethical Decision-Making Confidence, and Moral Courage Among Nurse Leaders

Background

Nurse leaders report low confidence navigating ethical dilemmas. Ethical dilemmas arise when ethical values are challenged, causing uncertainty about how to proceed. Left unresolved, ethical dilemmas impact ethical decision-making and can lead to moral distress. Moral distress is the anguish that occurs when there is recognition of an ethical dilemma necessitating a professional obligation to act, yet barriers exist to taking the ethically correct course of action.

To reduce moral distress, nurse leaders not only need to be able to identify an ethical dilemma, know …

Nitrous Oxide Administration In General Anesthesia Patients, Kaley Smith, Karen Colorafi, Kenn Daratha, Braden Hemingway Oct 2023

Nitrous Oxide Administration In General Anesthesia Patients, Kaley Smith, Karen Colorafi, Kenn Daratha, Braden Hemingway

Central Division Nurse Clinical Inquiry Conference

Abstract:

Background:

Nitrous oxide is an inexpensive and fast-acting gas used in a variety of inpatient and outpatient perioperative settings to hasten the onset of and emergence from general anesthesia. Problematically, nitrous oxide has been identified as a risk factor for postoperative nausea and vomiting (PONV), one of the most common adverse events after anesthesia. Unresolved PONV is linked to increased pain, wound dehiscence, aspiration, and longer stays in the recovery area and/or hospital. An Apfel Simplified Risk Score can be calculated to predict PONV based on four patient characteristics: female sex, non-smoker, history of PONV or motion sickness, and …

Rates Of Postoperative Pneumonia In Healthy Adults Receiving Neostigmine Versus Sugammadex: A Multi-Center Evidence-Based Practice Project, Kirstie Gorthy, Braden Hemingway, Karen Colorafi, Kenn Daratha Oct 2023

Rates Of Postoperative Pneumonia In Healthy Adults Receiving Neostigmine Versus Sugammadex: A Multi-Center Evidence-Based Practice Project, Kirstie Gorthy, Braden Hemingway, Karen Colorafi, Kenn Daratha

Central Division Nurse Clinical Inquiry Conference

Abstract:

Background

Residual neuromuscular blockade increases morbidity and mortality following surgery with general anesthesia and paralysis. Optimal antagonism of pharmacologic muscle relaxation is critical to avoiding residual weakness and subsequent pneumonia. The clinical question that guided the investigation of the evidence was, ‘Among healthy adults requiring neuromuscular blockade during general anesthesia for elective, noncardiac surgery, does the use of sugammadex compared to neostigmine for reversal of neuromuscular blockade influence the rate of postoperative pneumonia within seven days?’

Purpose

The primary purpose of this evidence-based practice project was to describe the rates of pneumonia among healthy adults antagonized with neostigmine or …

Leveraging Technology To Improve Assessment Of Acute Care Nurses' Learning Needs, Carsen Mcguire, Maria Heinje

Central Division Nurse Clinical Inquiry Conference

Abstract:

Background: Healthcare organizations pursuing the American Nurse Credentialing Center (ANCC) Magnet® Award must assess the educational needs of Registered Nurses and develop relevant educational offerings that positively affect patient outcomes. Challenges include collecting survey response rates, report build time, and accessible of reports to those responsible for the education. Four hospitals in the Pacific Northwest used electronic workflows to disseminate the educational needs of acute care nurses by unit, specialty, and role with improved analysis of data. Incorporating a learning platform for survey distribution increased the response rate from 15% to 70% over 3 years. The valid response rate …

Differences Between Nurses Who Provide A Gut Microbiome Sample For Research And Those Who Do Not, Teresa Rangel, Ellen Romesburg

Central Division Nurse Clinical Inquiry Conference

Abstract:

Title: Demographic differences between nurses who provide a gut microbiome sample for research and those who do not

Background: In the United States (U.S.), there are over 4.1 million registered nurses and 60% work in acute care to treat hospitalized patients. On average, the nursing workforce is female, Caucasian, and approximately 49 years old. Only 60% of U.S. nurses report a bachelor’s degree education or higher. Compared to other licensed care professionals, nurses suffer higher prevalence of obesity and are high risk for metabolic diseases which may negatively impact their occupational and physical wellbeing. The gut microbiome …

Comparison Of Dietary Intake, And Continuous Blood Glucose Measurement In Nurses Working Either 12-Hour Day Shifts Or Night Shifts, Sushana Sudhi, Andrew Browning, Megan Kevil, Teresa Rangel, Rachel Carlson, Lindsey Miller

Central Division Nurse Clinical Inquiry Conference

Abstract:

Title: Dietary intake and continuous blood glucose measurement in nurses working either 12-hour day shifts or night shifts

Sushana Sudhi, Andrew Browning, Megan Kevil, Teresa Rangel, Rachel Carlson, Lindsey Miller

Background:

Nursing night shift work is essential in the hospital yet is linked to negative outcomes such as cardiometabolic illnesses including type 2 diabetes. Short-term studies (lasting a few days) of simulated night shift work have identified night-time decreases in insulin sensitivity and glucose tolerance as potential causes of negative cardiometabolic health outcomes. However, the long-term effect of regular night shift work on glucose regulation among nurses remains unknown. …

Nurse Perceptions Of Providing Continuous Blood Glucose Data For A Research Study, Teresa Rangel, Lindsey Miller, Rachel Carlson Oct 2023

Nurse Perceptions Of Providing Continuous Blood Glucose Data For A Research Study, Teresa Rangel, Lindsey Miller, Rachel Carlson

Central Division Nurse Clinical Inquiry Conference

Abstract:

Title: Nurse Perceptions of Providing Continuous Blood Glucose Data for a Research Study

Background: Acute care nurses are essential to preserve quality and quantity of life of hospitalized patients. However, nurses present with a higher risk of type II diabetes compared to other licensed healthcare providers, especially if assigned to night shifts, despite controlling for modifiable lifestyle factors such as diet and exercise. Laboratory studies suggest that eating during the night hours may cause an aberrant glucose response. It is hypothesized that consistently eating a meal during night hours may provide one mechanism for increased risk of …

A Team Mentor Project To Reduce Turnover On A Cicu, Sarah Evans Oct 2023

A Team Mentor Project To Reduce Turnover On A Cicu, Sarah Evans

Central Division Nurse Clinical Inquiry Conference

Abstract:

Background

From 2022-2023, a 28-bed cardiac care intensive unit (CICU) employing 120 Registered Nurses (RNs) hired 63 RNs, including 3 new graduates and 32 new-to-specialty, following significant turnover. Nurses in the CICU frequently face complex situations, leading to emotional distress and potential turnover if coping resources are not provided. Research suggests that pairing a new CICU nurse with an experienced mentor to help cope with complex situations can significantly increase confidence and resiliency and reduce emotional distress and turnover. On our CICU, nurses are assigned a preceptor for 12-20 weeks to orient to CICU-specific competencies without an emphasis on …

Implementing A Mentorship Program On A Medical/Oncology Unit, Nicole Frye, Stephanie Perreiah, Jenna Vansanford

Central Division Nurse Clinical Inquiry Conference

Abstract:

Title: Implementing a Mentorship Program on a Medical/Oncology Unit Background: New-to-specialty acute care nurses are generally assigned a preceptor to demonstrate complex competencies within 90 days of hire. However, providing inpatient nursing care presents psychosocial stressors not well-addressed by preceptors which can contribute to burnout and turnover. Participating in a mentorship program addressing psychosocial nursing stressors following competency-focused preceptorship may reduce burnout and turnover. A medical-oncology unit of a 200-bed urban hospital lacked a mentorship program and Unit-Based Council (UBC) members worked to implement and evaluate this resource. Purpose: To evaluate barriers and facilitators of participation in a new …

Implementing In-Room Recycling On A Family Medical Center, Sarah Johnson Oct 2023

Implementing In-Room Recycling On A Family Medical Center, Sarah Johnson

Central Division Nurse Clinical Inquiry Conference

Abstract:

Title: Implementing In-Room Recycling on a Family Medical Center

Background:

Healthcare operations are estimated to generate up to 29lbs of waste per patient per day (Practice Greenhealth, 2022). Waste contributes significantly to healthcare’s carbon emissions, which in turn contributes to the growing climate crisis (Karliner et al., 2019, Healthcare Without Harm, 2019; World Health Organization, 2018). Diverting recyclable waste from healthcare operations will reduce healthcare’s total waste to landfill and waste related carbon emissions.

Recycling bin placement and contamination rates (the amount of items in recycling that should go to another waste stream) can impact recycling levels. Staff at …
