In 1997, the Population Council initiated the Horizons Program—a decade-long USAID-funded collaboration with the International Center for Research on Women, the International HIV/AIDS Alliance, PATH, Tulane University, Family Health International, and Johns Hopkins University—designing, implementing, evaluating, and expanding innovative strategies for HIV prevention and care. Horizons developed and tested ways to optimize HIV prevention, care, and treatment programs; worked to reduce stigma and improve gender-biased behaviors; and greatly expanded knowledge about the best ways to support, protect, and treat children affected by HIV and AIDS. In all its projects, Horizons strengthened the capacity of local institutions by providing support and …

The Population Council, in collaboration with the Brazil Ministry of Health, the Municipal Program of STD/AIDS of Campinas, and the US Centers for Disease Control and Prevention/Global AIDS Program/Brazil, conducted a study to characterize the sociodemographic, behavioral, and environmental factors associated with subgroups of MSM who are most at risk for HIV infection. This research is the first Brazilian study to provide population-based estimates for HIV, syphilis, and HIV-related risk behaviors using an innovative sampling technique to reach members of the target population. A study conducted by the Population Council in 2001 found that truckers crossing Brazil’s southern border had easy access to commercial sex and extremely limited access to health services, condoms, HIV testing and counseling, and HIV/STI prevention messages. In response, the Population Council’s Horizons Program conducted an intervention study from 2002 to 2005. The study examined the feasibility, acceptability, and impact of providing a range of health services to truckers at a health post inside a customs station, where truckers wait anywhere from one day to a week for documents and cargo to clear customs. The Horizons program and the Promundo Institute studied the effectiveness of interventions in Brazil designed to change the attitudes of young men in relation to gender norms and reducing the risk of contracting HIV/STIs. One conclusion of the study was the recognition of the importance of engaging young people (men and women) in the issue of gender relations and the risks of HIV contamination. The results of the study indicate that addressing inequitable gender norms, particularly those that define masculinity, can be an important element of HIV prevention strategies. These findings suggest that group education interventions can successfully influence young …

Research with mobile populations has demonstrated that men in the mobile workforce tend to be exposed to greater HIV risk, and have higher sexually transmitted infection (STI) and HIV prevalence, than those in less mobile or non-mobile professions. At the request of the Brazilian Ministry of Health and with support from USAID/Brazil, the Population Council conducted an assessment in Brazilian border areas to determine which populations were most in need of HIV prevention activities. There is growing evidence that social norms that promote unequal gender roles increase young men and women’s risk of HIV/STIs and partner violence. Thus, changing gender norms is increasingly recognized as an important strategy to prevent the spread of HIV infection. Few interventions to promote gender-equitable norms and behaviors among young men have been systematically implemented or evaluated, and little is known about how best to measure changes in gender norms and their effect on HIV/STI protective and risk behaviors. The female condom is a relatively new product that is intended to serve the dual role of protecting against unwanted pregnancy and sexually transmitted infections (STIs). Recent research has found moderate to high levels of initial trial and acceptance of the female condom among women. However, information is lacking about its continued use, particularly among women at high risk of HIV and other STIs. The female condom was registered in Brazil in January 1997 and since then has been available commercially through DKT, a social marketing organization. It examines how BEMFAM/Brazil, ASHONPLAFA/ Honduras, and FAMPLAN/Jamaica have made a transition toward a range of family planning services more focused on the clients themselves and more explicit in the treatment of sexual themes. This issue of Quality/Calidad/Qualité tells the story of a group of Brazilian women who came together in 1981 with the shared belief—not as widely understood then as it is now—that conventional approaches to women's health and family planning services were overly medicalized, incapable of dealing with the relationship between sexuality and contraception, and disempowering to clients. They started the Coletivo which runs a clinic, a training institute, and a public education program that has “given voice” to thousands of women, provided leadership to the Brazilian government and women’s health movement, and has been active in research and international policy affairs. …

The proposals in the article are derived from the experience of PRO-PATER, in São Paulo, Brazil, and briefly, from the activities of the Profamilia Men’s Clinic in Colombia.
