Problem Addressed and Project Purpose Motivated by the transition to value-based payment models, 1 there are growing efforts to capture information about patients’ social determinants of health (SDOH) in United States (US) healthcare and provide subsequent services targeted at those health-related social risk factors. Background

Data from remote monitoring (RM) of cardiovascular implantable electronic devices (CIEDs) currently are not accessible to patients despite demand. The typical RM report contains multiple pages of data for trained technicians to read and interpret and requires a patient-centered approach to be curated to meet individual user needs.

Objective

The purpose of this study was to understand which RM data elements are important to patients and to gain design insights for displaying meaningful data in a digital dashboard.

Methods

Research Objective Up to 80% of the factors contributing to an individual’s health status are social determinants of health (SDOH). As healthcare systems transition to value-based payment models which compensate providers for patient health outcomes, many have called for healthcare organizations to screen patients for SDOH-related social needs and for providers to consider them when providing medical care. Researchers have an opportunity to develop assistive applications that empower people to change unhealthy habits through monitoring their behavior. Mobile applications can enhance self-monitoring by providing real-time feedback and employing persuasive technology. The projects presented demonstrate the potential of persuasive, assistive applications for both chronically ill and healthy individuals.
