Patient physical inactivity during hospital admission is a key contributor to increased length of stay and poor outcomes. Patient inactivity and immobility in the intensive care unit (ICU) setting promote the loss of physical function and increase the risk of complications such as pressure injuries, bone demineralization, lung infections, and ICU delirium. The cumulative effects of immobility are persistent through discharge, as patients are at risk for the long-term effects of posthospitalization muscle atrophy and weakness. Hospital costs are the most significant portion of health-related costs incurred by non-profit health insurers registered as medical schemes in South Africa. Medical schemes continue to use retrospective reimbursement models for hospitals despite general acknowledgment that these models do not limit hospital costs and utilization. Although medical schemes are interested in implementing alternative reimbursement models, such as pay-for-performance (P4P), they are uncertain about which P4P models they can use to reduce hospital costs, resulting in their inability to make critical changes to their costs from traditional fee-for-service models. The demand for physician assistants (PAs) is predicted to rise because of the growing shortage of physicians. PA educational programs are tasked with producing graduates who are skilled within six domains of competency: 1) medical knowledge, 2) professionalism, 3) interpersonal and communication skills, 4) patient care, 5) practice-based learning and improvement, and 6) systems-based practice. The Physician Assistant National Certifying Exam only assesses two of the six competencies: medical knowledge and professionalism. Background and Purpose: Reduction of live hospice discharges in the form of revocations is necessitated in order to maintain regulatory compliance and meet quality measures. Upon admission to hospice services, many have not considered personal desire regarding further intervention beyond Do Not Resuscitate (DNR) status. Evidence-based practice has revealed Physician Order for Scope of Treatment (POST) forms as effective in enhancing translation of patient desires related to care and interventions at end-of-life.
