Due to the high risk of cardiovascular disease and other negative complications, it is imperative that patients in the primary care clinic diagnosed with hypertension control blood pressure levels. The purpose of this scholarly project was to increase the initiation of self-management goals (SMGs) among adult males and females age 30 to 60 diagnosed with hypertension. These patients were active duty military, reservists, and dependent beneficiaries who receive care at the Navy Medicine Readiness Training Unit–Memphis. Hypertension is a global chronic medical condition that is life threatening and eventually fatal if left untreated. Elevated blood pressure will disrupt the function of the cardiovascular system and impact the function of major organs, resulting in devastating complications for the patient while increasing cost for the healthcare system. This evidence-based quality improvement project is aimed at increasing control for veterans with high blood pressure and lowering blood pressure to a goal under 140/90 through a nurse driven education program. Low health literacy is closely correlated with poorly-managed chronic disease conditions. Hypertension is a common chronic disease condition which can be better controlled through compliance with the dietary approaches to stop hypertension (DASH) diet; however, patients with low health literacy may have difficulty understanding the DASH diet without proper education.
