Cost Outcomes For Major Depressive Disorder And Bipolar Disorder Across Professional License Types And Modalities, Julia H. Jones

The purpose of this study was to compare outcomes for patients with Bipolar Disorder or Major Depressive Disorder based on severity of diagnosis. This study also compared psychotherapy providers and therapy modalities on total cost, number of sessions, and dropout. Our data set (N=136,439) came from Cigna, a national health care company. Results showed significant differences by severity of diagnosis. The comparison of providers showed that psychologists had higher costs and session numbers, while the other providers were not significantly different. Hypogonadism is one of the most common endocrinologic syndromes seen in clinical practice. Testosterone replacement therapy (TRT) is one possible treatment option for men with hypogonadism. When TRT is a viable treatment option, routes of administration, effectiveness, patient compliance, cost, and potential side effects should be carefully considered by the Nurse Practitioner. In the United States TRT is available via intramuscular, nasal, buccal, and topical routes. All transdermal testosterone is the most effective when regulating testosterone levels. Depotestosterone, an injectable form of testosterone, promotes patient compliance, is considered safe, and is affordable. Nurse Practitioners should initiate TRT with Depotestosterone.

This paper presents results from cost-analysis research for treating Post-Traumatic Stress Disorder in adults. Data for this research was provided by Cigna, a nation-wide healthcare manager in the United States of America. The sample size was 12,845 adults who were diagnosed with PTSD and received outpatient individual and family psychotherapy from one of the following mental health care providers: Marriage and Family Therapists (MFTs), Master’s Nurses (RNs), Medical Doctors (MDs), Professional Counselors (LPCs), Psychologists (PSYs), and Social Workers (MSWs). Although a common psychological disorder, pharmacological treatment of Attention Deficit Hyperactivity Disorder can be a costly endeavor. Among 4-5 year olds, methylphenidate is a safe, efficacious, and cost-effective option for short-, intermediate-, and long-acting medication options. Short-acting methylphenidate, intermediate-acting methylphenidate ER and SR, and long-acting methylphenidate LA are the most efficacious and cost-effective medications. Selection of non-stimulant medications should be based upon recommended guidelines rather than primarily on cost. For children incapable of swallowing pills, the most efficacious and cost-effective treatments are short-acting dextroamphetamine liquid and long-acting methylphenidate LA capsules that can be opened and sprinkled over food.

Generalized anxiety disorder (GAD) is frequently found in primary care settings and is highly prevalent among adolescents. The purpose of this study was to examine the cost effectiveness by provider type and therapy modality in treating adolescents (ages 13-17) with a GAD diagnosis (DSM-IV 300.02). A national insurance company in the United States provided outpatient and unidentifiable data for adolescent GAD cases (n = 2,932). These cases were used to analyze the cost effectiveness, total cost, treatment length, dropout, and readmission rates for the treatment of adolescents with GAD. Background: Central line–associated bloodstream infections (CLABSIs) result in increased length of stay, cost, and patient morbidity and mortality. One CLABSI prevention method is disinfection of intravenous access points. The literature suggests that placing disinfectant caps over needleless connectors decreases CLABSI risk.

Methods: A quasi-experimental intervention study was conducted in a >430-bed trauma I center. In addition to an existing standard central line bundle, a new intervention consisting of a luer-lock disinfectant cap with 70% alcohol was implemented in all intravenous (IV) needleless connectors on patients with peripheral and central lines. Mood disorders are often linked with concurrent partner relational distress. The present study compared the cost effectiveness of treating mood disorder alone versus when the condition is comorbid with partner relational distress. Cigna, a leading health insurance management company in the US, provided outpatient data. Participants included patients with solely a mood disorder diagnosis (n = 72,712) and those with both a mood disorder and a comorbid partner relational distress diagnosis (n = 113, including 69 females and 44 males). High-fidelity simulation (HFS) is sweeping the nation as a legitimate and exciting new resource for nursing educators. As with most new technologies, HFS is not without its difficulties. It can be a costly program to run and may also cause anxiety among the faculty required for implementation. There is a simple solution to help increase the ease of its implementation: using student workers. This article discusses how using students to help run the simulations can not only reduce overall cost and anxiety associated with this new technology but also increase learning among those participating in HFS.
