Pharmacotherapy For Non-Alcoholic Steatohepatitis - Current Status And Future Prospects, Ganesh Bhat

Manipal Journal of Medical Sciences

Non-alcoholic Steatohepatitis (NASH) is emerging as a leading cause of cirrhosis worldwide. Weight reduction through lifestyle modification is accepted first line treatment for these patients. Histological improvement in highrisk patient is the primary target of pharmacotherapy in NASH patients. Even though currently many drugs are available for these high-risk patients, none of them has been approved USA Food and Drug Administration (FDA) until now. Among these, vitamin E and pioglitazone has been approved by AASLD (American Association for the Study of Liver Diseases) in selected group of patients. There are more than a dozen drug targeting different pathways in NASH …

Post-Coarctectomy Syndrome And The Use Of Gabapentin For Gi Ileus: A Case Report, Vinay Krupadev Dec 2021

Post-Coarctectomy Syndrome And The Use Of Gabapentin For Gi Ileus: A Case Report, Vinay Krupadev

Manipal Journal of Medical Sciences

Post-coarctectomy syndrome is a complication of aortic arch repair that occurs due to a sudden increase in reperfusion of the body’s vasculature following surgery. One complication related to postcoarctectomy syndrome is ileus which can prolong a patient’s ICU stay and delay discharge home. While traditionally supportive care has been used to resolve this condition, this case highlights the use of gabapentin as a relatively novel treatment for post-coarctectomy syndrome gastrointestinal ileus.

Post-Operative Vision Loss Following Cataract Surgery: Lessons Learned From An Eight-Year Analysis, Thanigasalam Thevi Dec 2021

Post-Operative Vision Loss Following Cataract Surgery: Lessons Learned From An Eight-Year Analysis, Thanigasalam Thevi

Manipal Journal of Medical Sciences

Background: Cataract is an ageing process and surgery is the only mode of restoring vision. Patients who undergo surgery have only one aim in mind, which is improvement in their visual acuity (VA). However, some patients do not achieve this aim for improved vision after having undergone the surgery. Reasons for worsening vision after cataract surgery should be identified to counsel the patients on the possibility of poorer outcomes so that patients are prepared for such eventualities should they happen. Objective: While cataract surgery restores vision in most, a subset of patients experience worsening vision post-operatively. We investigated various reasons …

Percutaneous Tracheostomy; As It Stands Today, Devaraja K

Manipal Journal of Medical Sciences

Conventionally, tracheostomy includes surgical creation of tracheo-cutaneous communication. It is done either to bypass the upper airway obstruction or to facilitate optimal mechanical ventilation and early weaning off the ventilator support in neurologically unstable patients. For many years now, percutaneous tracheostomy has been proposed as an alternative to open surgical tracheostomy in ventilator-dependent patients. This review briefly narrates the principles and procedure of percutaneous tracheostomy. In addition, the article sheds light on a few critical problems that one could encounter during the process and discusses countermeasures to overcome those difficulties. Finally, the role of intra-procedure bronchoscopy or ultrasonography and of …

Malabar Pit Viper Envenomation Presenting With Symptomatic Hyperkalaemia In A Tertiary Care Centre In South Karnataka, Vrinda Lath Dec 2021

Malabar Pit Viper Envenomation Presenting With Symptomatic Hyperkalaemia In A Tertiary Care Centre In South Karnataka, Vrinda Lath

Manipal Journal of Medical Sciences

A 25-year-old male was bitten by a pit viper over the foot and presented with a tender and oedematous bitten limb a day after the bite. He had received anti-snake venom (ASV) at a local centre and developed anaphylaxis. He was admitted for observation and developed sudden onset hyperkalaemia and hemodynamic instability which responded to anti-hyperkalemic measures. We highlight envenomation by a species not known to cause any life-threatening complications, and emphasize the need for intensive monitoring and discuss the importance of species identification, knowledge of regional herpetofauna and treatment modalities including the need for a species-specific antivenin.

A Rare Case Of Chorioretinitis Sclopetaria- A Pellet In A Haystack, Yogish Kamath

Manipal Journal of Medical Sciences

Chorioretinitis sclopetaria is a rare event with a poor visual prognosis, with less than a hundred cases reported worldwide. We report a case of an 8-year-old boy who presented with swelling and decreased vision in the left eye after an alleged fall. On examination, the affected eye showed periorbital ecchymosis, ptosis, subconjunctival haemorrhage, hyphema, and an afferent pupillary defect. A CT scan showed a foreign object lodged in the left orbit which was removed surgically. This case report intends to describe the rare occurrence of chorioretinitis sclopetaria in a child, wherein the history of gun pellet injury, elicited in retrospect, …
