Accutane is a pill-based derivative of Vitamin A used to treat cystic acne. The process to start and continue the medication each month is tedious and full of potential for error. This is especially true for female patients who are able to become pregnant as they have the additional step of monitoring and updating their birth control through an online portal. Patients are treated like numbers and there is little to no customization within Accutane treatment from patient to patient. Acne is one of the most common skin disorders in the United States, occuring in over 5.1 million Americans. Of those with acne, 53.8% are adults between the ages of 18 and 44 years old (American Academy of Dermatology [AAD], 2017). Acne can lead to multiple issues such as scarring, poor self-esteem, anxiety, and depression (Zaenglein et al., 2016). Acne can also negatively impact quality of life (Cengiz & Gurel, 2020). A common struggle for many young adults is the infamous acne vulgaris. Americans spend millions of dollars on acne medication, treatments, and dermatology and cosmetic visits each year. One area that is greatly lacking in research is the relation of nutrition and facial skin appearance. OBJECTIVE: The objective of this selective EBM review is to determine whether or not “Is spironolactone an effective method for reducing lesion count in individuals with acne vulgaris?”

STUDY DESIGN: A systematic review of three English language studies published after 2009 in peer-reviewed journals, including two randomized, controlled trials and one retrospective cohort study.

DATA SOURCES: Two randomized, controlled trials and one retrospective cohort study that evaluated spironolactone as a treatment for acne vulgaris were found using PubMed.

OUTCOMES MEASURED: Reductions in total lesion counts (TLC).

Acne is a prevalent skin disorder and is displayed through various different incidences. Cystic acne, a severe form of acne that is difficult to treat, reaches deep into the skin underneath the epidermis layer and produces painful, inflamed cysts that contain sebum, dead skin cells, and Propionobacterium acnes, the acne causing bacteria. Objective: The objective of this selective EBM review is to determine whether or not the use of oral isotretinoin is more effective than oral antibiotics in the treatment of severe acne.

Study Design: Review of all English language primary studies published after year 2000.

Data Sources: Three randomized control trials all found using PubMed. Outcomes Measured: Total number and percentage change in total acne lesions from baseline at various intervals.

OBJECTIVE: The objective of this selective EBM review is to determine whether or not a once daily topical application of fixed-dose combination gel containing adapalene 0.1%-benzoyl peroxide (BPO) 2.5% (adapalene-BPO) is effective at treating mild to moderate acne.

STUDY DESIGN: Three multi-center double blind randomized control studies published in the English language between 2007-2013.

STUDY SOURCES: Three peer-reviewed RCTS were found using Pubmed. These studies compared the same fixed-dose combination gel adapalene-BPO against placebo vehicles. Two studies also compared to additional groups receiving monotherapies.
