The goal of the dissertation is to undertake an analysis of substance use disorders that focuses on a public health ethics perspective. The ethical argument focuses upon justifying the use of harm reduction and is primarily concerned with the current opioid crisis. While substance abuse/misuse over the course of history has been identified as a public health concern, this dissertation presents substance use disorders over the course of the lifespan, examining various contexts in which it can affect daily living and health outcomes. Abstract

The United States is in an opioid epidemic. The comitant use of opiates and benzodiazepines can ultimately result in death. This project consisted of a quality improvement approach addressing the continued need to educate prescribers on coprescribing benzodiazepines and opioids to the adult population. A qualitative and quantitative descriptive analysis was utilized for data collection. The results will assist in determining if accessing the Prescription Drug Monitoring Program (PDMP) and utilizing evidence-based guidelines before prescribing treatment compared to the nonutilization of the PDMP and evidence-based guidelines aid in the reduction of mortality and overdose rates within a three-month period. …

Background

Research suggests prolonged postoperative opioid use occurs in 4-13% of opioid naïve patients and is related to factors other than surgical pain. However, it is unclear which patient factors and prescribing practices are associated with prolonged use after surgery among opioid naïve patients.

Objectives

To identify factors associated with prolonged postoperative opioid use (refills 90-180 days after surgery) in opioid naïve patients in two domains: specific patient characteristics (demographics, smoking status, comorbidities, etc.) and exposure through postoperative opioid prescriptions (in oral morphine milligram equivalents [OME]).
