Since the COVID-19 pandemic, there has been an upswing in the lack of awareness concerning disparities and the lack of resources available in many of the neighborhoods in the Chicagoland area. The Washington Park and Bronzeville areas are two (2) communities among several others in the Chicago area that are considered to be low-income neighborhoods with minimal resources and an increasing concern of disparities. Type 2 diabetes continues to affect millions of people every day across the United States. Uncontrolled blood sugars pose a risk to individuals’ overall health by causing microvascular and macrovascular changes and ultimately lead to an earlier mortality. One’s blood sugar levels can be checked at any time, but an individual’s hemoglobin (hgb) A1c is a three-month average that allows the diabetic to have an overview on how they are managing their diabetes. The prevalence of diabetes continues to rise, but patients’ ability to obtain and maintain glycemic control remains a challenge. In 2017, complications from diabetes resulted in over $300 billion expenditures in lost productivity and medical costs. The nation’s approach to addressing this problem starts with acknowledging the benefits that continuous glucose monitoring (CGM) as an adjunct intervention could change the trajectory of both diabetic care outcomes and the national expenditures associated with poor glycemic control, which leads to the destruction of other cells such as those found in the heart, eyes, and kidneys. Hypoglycemia can cause an array of outcomes including increased morbidity, mortality, and hospital length of stay. The literature revealed specific educational methods effective in decreasing hypoglycemia including case-based learning tutorials and unit-based lectures. Focused nursing education regarding the cause of hypoglycemia and understanding the importance of provider notification of hypoglycemia was also shown in the literature to reduce rates of hypoglycemia. The purpose of this project was to decrease inpatient hypoglycemia incidence utilizing a unit-based educational seminar as an intervention on a diabetic renal unit within a hospital. Billions of dollars are spent on diabetes in healthcare each year. Patients with lower socio-economic-status tend to have poor glycemic control, higher associated diabetic complications, and higher associated mortality rates. In underserved patients with type 2 diabetes, does utilization of a Diabetes app, compared to previous self-care management and clinic education, lower the Hemoglobin A1c over a 3-month period? A 3-month scholarly project was conducted in a small rural town in Virginia with an average median income around $17,000. N=21 were selected for the scholarly project.
