Background: Medical education changed due to the COVID-19 pandemic. Educational sessions were offered virtually. There are published education innovations for pre-clinical curricula (1). However, providing virtual education during the clinical clerkship was a daunting challenge. In addition to the delay in patient care exposure, students’ performance in the NBME Subject Matter (Shelf) Exam scores have declined during the pandemic (2). VCU School of Medicine also needed to provide continued instructions to students whose clinical clerkships were delayed.

Purpose and background, including relevant citations: All pediatric residents are required to rotate through the neonatal intensive care unit (NICU). The NICU is a unique unit that provides care for premature and ill neonates. At Children’s Hospital of Richmond at VCU (CHOR VCU), pediatric residents complete 12 weeks in the NICU, 4 weeks each year. As a part of this rotation, these residents are responsible for attending high risk deliveries. At birth about 10% of neonates will require resuscitation (1). Before starting residency, each resident is required to complete the neonatal resuscitation program (NRP). This certification requires renewal every two …

Quiet Please! The effects on sleep quality and quantity as a result of QI project to minimize nighttime interruptions

INTRODUCTION: Sleep is a significant component of health that is often overlooked in hospitalized patients. We introduced a QI project in 2016 to minimize nighttime interruptions in clinically stable patients by performing “passive vitals” which eliminated temperature and blood pressure readings at 4am and allowed patients to sleep longer overnight. Despite this intervention, the effects on sleep quality and quantity are unknown.

Background: The Surviving Sepsis Campaign recommends initiating IV antibiotic administration within one hour of recognition of severe sepsis. Several studies have shown that prompt blood culture collection, administration of broad-spectrum antibiotics, and fluid resuscitation following recognition improves child survival.

Objective: Our goal was to evaluate effectiveness of sepsis initiatives and institutional changes in the timing of early antibiotic administration at Children’s Hospital of Richmond at VCU.

Background
Identification of factors associated with antifungal utilization in neonatal, pediatric, and adult patient groups is needed to guide antifungal stewardship initiatives in academic medical centers.

Methods
For this hospital-level analysis, we analyzed antifungal use in hospitals across the United States of America, excluding centers only providing care for hematology/oncology patients. Analysis of variance was used to compare antifungal use between patient groups. Three multivariable linear regression models were used to determine independent factors associated with antifungal use in the neonatal, pediatric, and adult patient groups.

Physicians are expected to be leaders. Pediatrics residents have identified a lack of leadership development in their training. Some residency programs have developed skills-based leadership curriculums that are perceived positively by participants. A leadership curriculum is being developed for VCU pediatrics residents in a fellowship track. A needs assessment was done to identify high-priority leadership skills. The curriculum aims to improve communication, conflict resolution, decision-making, and time management skills using role-play, reflection, and feedback from direct observation. BACKGROUND: Using management algorithms or pathways potentially improves clinical outcomes. We undertook systematic reviews to examine various aspects in the generic approach (use of cough algorithms and tests) to the management of chronic cough in children (aged ≤ 14 years) based on key questions (KQs) using the Population, Intervention, Comparison, Outcome format.

METHODS: We used the CHEST Expert Cough Panel's protocol for the systematic reviews and the American College of Chest Physicians (CHEST) methodological guidelines and Grading of Recommendations Assessment, Development and Evaluation framework.
