Background: At-risk drinking is common in Australia. Validated screening tools such as the AUDIT-C have been promoted to general practitioners (GPs), but appear rarely used and detection of at-risk drinking in primary care remains low. We sought to describe Australian GP perceptions of the detection and screening of at-risk drinking; to understand their low uptake of alcohol screening questionnaires, and in particular, their attitude to the adoption of the AUDIT-C. Ever since Dr Charles Butterworth's seminal article in 19741 on the widespread prevalence of malnutrition in hospitalised patients and its detrimental effect on recovery, attempts have been made to improve the detection of malnutrition through screening initiatives. Objective: To validate a two-item food security questionnaire (FSQ) for use in a clinical setting to screen HIV-1 infected patients for food insecurity. Design: The present study was a questionnaire-based survey of forty-nine subjects attending an HIV clinic. Subjects completed a two-item questionnaire and a six-item validated FSQ contemporaneously. Results: A strong correlation was found between the two-item and six-item FSQ (r50?895; 95% CI 0?821, 0?940; P,0?0001). The counterfactual priming effect occurs when "what if" and "if only" thoughts about one event broadens ones mind-set to consider a more diverse range of evidence and possibilities in a different decision making or problem solving context. The present study tested the prediction that counterfactual thinking (thoughts of if only&) provides a vivid means for women to imagine whatcould have been done differently in hypothetical breast cancer scenarios for the protagonist to avoid their predicament.This should then encourage themto adopt a more preventative approach to and take greater personal responsibility toward their own breast health.Women aged 50 and older (N=181) read either a standard pamphlet on mammography rescreening or one containing counterfactually framed scenarios.The latter depicted fictitiouswomen whose failure to have routine mammograms contributed to their diagnosis with advance-stage breast cancer.The counterfactual group subsequently indicated greater feelings …

Objective: To develop a nutrition screening tool for use in older South Africans. Design: A cross-sectional validation study in 283 free-living and institutionalised black South Africans (60+ years). Methods: Trained fieldworkers administered a 24-hour recall and the Mini Nutritional Assessment (MNA) screening tool, and performed anthropometric measurements and physical function tests. Cognitive function was assessed using a validated version of the Six-Item Cognitive Impairment Test. Biochemical indicators assessed included serum albumin, haemoglobin, ferritin, vitamin B12, red-blood-cell folate, cholesterol and vitamin C. The prevalence of malnutrition within hospital settings is a major concern to all health care workers. The recent development of a simple screening tool for use in such settings has increased the opportunity to identify at-risk patients in a reasonable time frame during their admission. This paper outlines the implementation of a routine nutrition screening and assessment, performed completely by dietitians, across both acute and rehabilitation settings. Dietitians were able to screen, on average, 72% of eligible patients, which ensured timely dietetic intervention. The routine malnutrition screening and assessment process highlighted differences (P < 0.01) in the rates of malnutrition between the acute wards (range 7 to 14%) and rehabilitation ward (49%). Significant differences between acute and rehabilitation patients were also found within the majority of individual diagnostic groups, including all surgery, fractures, cardiovascular incidents and respiratory illness (P < 0.01). The identification of rates of malnutrition between different wards, diagnoses and institutional settings provides dietetic managers with a sophisticated tool that can assist in the allocation of dietetic resources. This operational framework for routine screening of nutritionally at-risk patients in hospital, enables dietitians to develop patient outcomes and an effective nutrition care model.
