Patients with anemia and other conditions resulting in loss of blood often come with orders to return wasted blood to them. While using typical blood draw practices, patients are placed at an increased risk of infection when their blood is exposed to the outside world. My question is, will using a closed system decrease the instances of infection in patients who receive their waste blood after draws? By utilizing a waste syringe attached to the IV line, nurses can avoid detaching and reattaching syringes to collect and give back the patient's blood. The primary purpose of this qualitative improvement project was to increase frequency of photos by bedside staff leading to a picture-perfect view and a story of the patient’s skin. Additionally, the purpose included:

  • Decrease HAPI rate in STICU
  • Develop Higher Standard of Care r/t Skin
  • Educate patients and families on importance of skin r/t illness while hospitalized

Problem: According to the American Association of Critical-Care Nurses (AACN), moral distress (MD) is a major issue in the healthcare profession (2015). Many healthcare workers (HCW) are not aware of MD. The inability to recognize MD may lead to departing the profession.

Purpose: To offer an educational workshop about MD 1) explore the frequency and level of MD (2) establish a decision tree to mitigate MD.

Background: MD occurs when a person understands what they should do but cannot, due to organizational policies. The pandemic has exacerbated MD.

The World Health Organization (W.H.O) recommends exclusive breastfeeding for the first six months of an infant’s life. A large majority of new mothers are making the decision to breastfeed their newborn child. According to the Centers for Disease Control and Prevention, as of 2016, high breastfeeding initiation rates show that most mothers in the U.S. want to breastfeed and are trying to do so. The Glasgow Coma Scale (GCS) was designed for use in a wide range of clinical environments with staff that had no special training (Teasdale and Jennett 1974). Evidence (Waterhouse 2007 and 2008) suggests however that there are ongoing problems with the use of the GCS assessment, and the impact that this might have on patient outcome is unknown. Variations in practice have been found in all areas, including the neurological specialties wards, however the skill level of nursing staff in non neurological areas is concerning with inconsistency in practice.
