The Audiology Oath: A Survey Of Opinions And Current Practices., James Steiger

We surveyed audiologists and students to elicit opinions and information regarding the Audiology Oath. Fewer than half of respondents were aware of the oath. The oath was shared with all surveyed; over 85% of respondents agreed with the components of the oath and less than 13% offered changes. White coat and graduation ceremonies are considered appropriate for oath recitation. We hope that whenever the Audiology Oath is recited it will strengthen commitment to ethical practice.

Assessment Of Objective Pulsatile Tinnitus In A Patient With Syringohydromyelia, James Steiger

We examined a 38-year-old male with syringohydromyelia and concomitant symptoms suggestive of intracranial hypertension including unilateral low-frequency sensorineural hearing loss and objective pulsatile tinnitus. The tinnitus was heard by the authors (through a hearing aid stethoscope tube), measured objectively (with a probe microphone), measured subjectively (as minimum masking levels and with fixed frequency Bekesy), and altered by changes in ear canal pressure (subjectively reported and measured objectively with a probe microphone). The audiologic symptoms were likely associated with elevated cerebrospinal fluid pressure that traveled to the cochlea through the cochlear aqueduct. Differential sensitivity for frequency was compared between six native speakers of a tone language, Mandarin Chinese, and six native speakers of a nontone language, English. Subjects judged whether variable tones, at increments within the frequency range of a level tone-phoneme category, sounded the same or different in pitch than standard tones at 1000Hz and 125 Hz. The audiologist's primary roles in healthcare include the identification of hearing loss, habilitation or rehabilitation, and hearing conservation. However, audiologists also function as gatekeepers to medical care. Appropriate medical referrals can, for some patients, reduce the risk of worsening hearing and other medical complications. It is therefore recommended that audiologists adopt clinic referral guidelines. This article contains guidelines for audiology that can be adopted as written or after editing. The guidelines can also be adapted for use in non-audiology clinics, including those of board-certified hearing instrument specialists. This article is divided into three sections. Although it has been reported that smoking has deleterious effects on the middle ears of smokers, including their eustachian tubes, investigators have failed to find differences in the tympanometric characteristics of smokers and nonsmokers. In the present study, however, we found that the Valsalva-induced tympanometric peak pressure shifts of young smokers were less than those measured among young nonsmokers. A similar but less compelling finding was observed among elderly subjects.

Researchers describe Mandarin Chinese tone phonemes by their fundamental frequency (Fo) contours. However, tone phonemes are also comprised of higher harmonics that also may cue tone phonemes. We measured identification thresholds of acoustically filtered tone phonemes and found that higher harmonics, including resolved harmonics above the Fo and unresolved harmonics, cued tone phonemes. Resolved harmonics cued tone phonemes at lower intensity levels suggesting they are more practical tone-phoneme cues in everyday speech. The clear implication is that researchers should use the Fo only as a benchmark when describing tone-phoneme contours, recognizing that higher harmonics also cue tone phonemes. These results …

Hearing Handicap Among Adult Residents Of An Urban Homeless Shelter, James Steiger

This retrospective study was undertaken to identify the prevalence of hearing loss in the homeless population and its implications for vocational rehabilitation. Audiometric threshold data for adult residents of an urban homeless shelter were collected and reported. Subjects with hearing loss were identified and defined by their binaural high-frequency pure tone average (B-HFPTA). Those subjects were assigned a predicted Hearing Handicap Inventory for Adults-Screener (HHIA-S) score. Their HHIA-S scores, in turn, were used to predict hearing handicap and hearing aid candidacy. Significant hearing handicap was predicted for 35.6% of subjects; 10.6% were identified as hearing aid candidates. These findings have …
