Spontaneous coronary artery dissection (SCAD) is a rare but potentially lethal event.1,2 It is a separation between the intimal and medial wall of the coronary vessel. The potential result of the dissection is obstruction of the vessel lumen and subsequent myocardial ischemia. Prior to the widespread use of coronary angiography, diagnosis was made at autopsy.2 From reported cases, some trends have been identified. There are reported associations with female sex,2,3 exercise,4–7pregnancy and postpartum state,8–12 coronary artery disease,2, 13, 14 and connective tissue disorders.9,15,16 The left anterior descending artery is more affected in women; the right coronary artery is more often …

Increasingly, drug-resistant Streptococcus pneumoniae is an identified pathogen causing pneumonia, sepsis, and meningitis. Our study evaluated outcome variables of patients infected with strains of S. pneumoniae moderately resistant to penicillin. We retrospectively analyzed the data on 110 Huntington, West Virginia, community patients who had types 6, 9, 14, 19, and 23 pneumococcal infections between 1982 and 1994. These strains are most likely to demonstrate resistance. Comparing patients infected with sensitive vs. moderately resistant pneumococcal strains, we found no significant difference in days in the hospital (10.9 vs. 11.1, P =.96), days of fever (2.4 vs. 2.8, P =.74), or mortality …

Several national studies have shown poor compliance with National Cholesterol Education Program II (NCEP) goals. A study we conducted of patients in the General Internal Medicine Clinic at the Marshall University Joan C. Edwards School of Medicine in Huntington showed that 46% of them were not at NCEP goals. We hypothesized that both patient and physician barriers were responsible for these findings so we administered two surveys about barriers to cholesterol management to 261 random patients identified with hypercholesterolemia and to all 50 residents and faculty at the clinic.
