OBJECTIVE: To examine differences in resident operative experience between male and female general surgery residents.
BACKGROUND: Despite increasing female representation in surgery, sex and gender disparities in residency experience continue to exist. The operative volume of male and female general surgery residents has not been compared on a multi-institutional level.
METHODS: Demographic characteristics and case logs were obtained for categorical general surgery graduates between 2010 and 2020 from the US Resident OPerative Experience Consortium database. Univariable, multivariable, and linear regression analyses were performed to compare differences in operative experience between male and female residents.

U.S. Counties With Higher Drug Overdose Rates Have Lower School Test Scores, Rajeev Darolia, Sam Owens, John Tyler

The impacts of the contemporary U.S. drug overdose crisis on child and family wellbeing have been profound. This brief describes the link between county-level opioid overdose rates and children’s test scores, finding that counties with higher overdose rates have lower average 3rd and 8th-grade test scores than counties with lower overdose rates. The relationship between higher overdose rates and lower test scores is particularly strong in rural counties. The places with the highest overdose rates and lowest test scores tend to be economically-disadvantaged, suggesting that economic investments may be needed to address the issue.

The adverse impacts of the U.S. opioid crisis have been documented in many domains, but surprisingly little attention has been directed to understanding how the opioid crisis has affected children’s educational outcomes. This brief shows that students in counties with high levels of opioid prescribing are learning more slowly over time than their peers in counties with low levels of opioid prescribing. In addition to directing more support to schools, the authors advocate for policies that address the underlying social conditions that lead to prescription opioid misuse.

The fields of genetics and genomics have greatly expanded across medicine through the development of new technologies that have revealed genetic contributions to a wide array of traits and diseases. This systematic review synthesizes research published from January 2010-July 2022 on the social determinants of ideal cardiovascular health (CVH) carried out around the world and compares trends in high-income countries (HICs) to those in low- and middle-income countries (LMICs). 41 studies met inclusion criteria (n = 28 HICs, n = 13 LMICs). Most were from the United States (n = 22) and cross-sectional (n = 33), and nearly all evaluated associations among adults. Among studies conducted in LMICs, nearly all were from middle-income countries and only one was carried out in low-income country. Education (n = …

I chose to solve a training and continuing education problem in my workplace, by creating a manual on burn surgery. This manual will later be turned into a computer education module, but will always be available in a hard copy as a reference material. I had to find an approach that reflected how my peers wanted to be educated and on what education points they felt were the most important to their success in surgery.

Vaccine hesitancy is a public health issue of growing concern. Extensive pre-existing literature offers several explanations and conflicting viewpoints regarding reluctance toward vaccinations. However, minimal research has been done exploring the upstream social determinants driving vaccine hesitancy. Utilizing academic and gray literature and interviews with experts in the field, this study addresses this gap in knowledge with the research question: To what extent does privilege impact vaccine hesitancy and resistance? Vaccine hesitancy appears globally and is particularly concerning given the re-emergence of vaccine-preventable diseases (VPDs). Introduction:
E-cigarette ever use has risen significantly in recent years in Ireland, similar to trends elsewhere in Europe, the United States, and Asia-Pacific region. Results from ESPAD Ireland (European School Survey Project on Alcohol and other Drugs) show teenage e-cigarette ever use increased from 18% (2015) to 37% (2019). Given this increase, our aim is to profile e-cigarette ever users and never users in this age group; to examine sociodemographic, personal, peer, and familial factors associated with e-cigarette ever use; and to suggest appropriate measures to reduce use.

Ensuring that mental health professionals are appropriately trained to provide affirming and sensitive care to transgender and gender diverse (TGD) adults is one mechanism that may reduce the marginalization sometimes experienced by TGD adults in mental health contexts. In this study, mental health professionals (n = 142) completed an online survey documenting the sources and types of training received to provide TGD-sensitive care; and, shared a self-assessment of their comfort, competence, and ability to provide TGD-sensitive care. This brief summarizes key results from the endline evaluation of the More than Brides Alliance (MTBA) project “Marriage: No Child’s Play” (MNCP) in India, Malawi, Mali, and Niger. The MTBA consists of partners Save the Children Netherlands, Simavi, Oxfam Novib, and the Population Council, along with 25 local implementing partners. The MNCP project—which took place from 2016 to 2020—aimed at being holistic and targeting pathways to child marriage on multiple levels simultaneously, treating communities as either having the full MNCP package or no intervention. This chapter investigates how this process of reconfiguring the “social” plays out in the context of the Philippines’ labor export system and pervasive culture of emigration. Focusing on the case of Filipino nursing graduates seeking to work overseas, this chapter discusses how the success of the Philippines’ labor-brokering process relies on individuals who can take on the responsibility of transforming themselves, mainly through education and training, into desirable workers for future employers. While the migration literature had largely framed emigration as an individual aspiration and project, this chapter demonstrates how families subsidize the Philippine state’s labor export system by taking …

As a guiding program theory, asset-building centers on the idea that skills, knowledge, relationships, and concrete resources can all constitute assets, or “stores of value,” that girls can, in turn, mobilize to make healthy choices, seek support, navigate institutions, and access entitlements. This approach is inherently multisectoral, rooted in a commitment to prioritize understanding of and respond to the diversity of girls’ needs, capacities, and experiences. The Asset Exercise operationalizes the concept of “asset-building” into concrete terms. OBJECTIVES: The prevalence of Alzheimer's disease and associated disorders is increasing. Rural residents in the United States have less access to memory care specialists and educational and community resources than in other areas of the country. Over a decade ago, we initiated an interdisciplinary rural caregiving telemedicine program to reach Kentucky residents in areas of the state where resources for supporting individuals with dementia are limited. Over the last several decades, global efforts to end female genital mutilation/cutting (FGM/C) have intensified through the combined efforts of international and nongovernmental organizations (NGOs), governments, and religious and civil society groups. Evidence of the wider impacts of FGM/C and interventions for its abandonment is small but emerging. The practice of FGM/C has frequently been linked to a girl’s marriageability and is thought to be associated with child marriage, either directly, as a cause of early/child marriage, or vice versa, or indirectly, resulting from common causes. The positive association between educational attainment and adult health (“the gradient”) is stronger in some areas of the United States than in others. Explanations for the geographic pattern have not been rigorously investigated. Grounded in a contextual and life-course perspective, the aim of this study is to assess childhood circumstances (e.g., childhood health, compulsory schooling laws) and adult circumstances (e.g., wealth, lifestyles, economic policies) as potential explanations. Background: Conflicting results have been reported regarding employment status and work ability in adults with congenital heart disease (CHD). Since this is an impor‐ tant determinant for quality of life, we assessed this in a large international adult CHD cohort. Methods: Data from 4028 adults with CHD (53% women) from 15 different countries were collected by a uniform survey in the cross‐sectional APPROACH International Study. Predictors for employment and work limitations were studied using general linear mixed models. Results: Median age was 32 years (IQR 25‐42) and 94% of patients had at least a high school degree. The More Than Brides Alliance (MTBA) implements the “Marriage: No Child’s Play” program (2016–20) in India, Malawi, Mali, Niger, and Pakistan. The MTBA is a holistic program that addresses early marriage through multiple strategies, including: sexual and reproductive health and rights, livelihoods, education, and prevalent community social norms around marriage. Furthermore, the program operates at multiple levels (girl, family, community, block, and district) to address individual, familial, and structural barriers to reducing child marriage in these communities. The More than Brides Alliance (MTBA) was formed to bringing together the strengths of a diverse team skilled in adolescent program implementation and research. The goal of the research presented in this report is to promote an evidence-based programmatic approach to delaying marriage in India, Malawi, Mali, Niger, and Pakistan by gathering detailed quantitative and qualitative information from the program areas over time to examine the impact of the present intervention and inform future strategies. Bangladesh ranks among the five countries with the highest rate of child marriage in the world. Community norms and values are among the underlying factors that support the practice of child marriage in Bangladesh. The Population Council is partnering with the Ministry of Women and Children Affairs to delay marriage as part of the UNFPA-UNICEF Global Program to Accelerate Action to End Child Marriage. This program is being implemented to generate evidence on reduction of child marriage through an adolescent skills-building program. Education on sexuality and HIV can help adolescents develop the capacity for healthy, respectful relationships and protect themselves from unwanted and unsafe sex, unintended pregnancy, and sexually transmitted infections. A 2015 Population Council study found that sexuality and HIV education programs that address gender and power in intimate relationships are five times more likely to be effective than programs that do not. However, most curricula still do not address these issues.
