Each year, Americans spend more money on health care than any other industrialized nation, despite comparable mortality rates for people with risk factors for heart disease. The reasons for this lack of health care value in the US are numerous and complex – including market distortions like supplier-inflated pricing and regulatory structures that enable consumers to utilize ubiquitous, high-cost medical technologies that yield uncertain benefits. Introduction

The needs assessment report discusses the effects of COVID-19 (Coronavirus) pandemic on a low-income Hispanic/Latino population specifically for diabetic patients seeking food pharmacy services at a non-profit clinic in San Mateo County. Nutrition is an essential aspect of diabetes management, which can be challenging to obtain among low-income populations. The COVID-19 pandemic has caused additional challenges among the general population, making it more challenging for the diabetic low-income population. This project was undertaken to identify the effects COVID-19 has on the food pharmacy patients.

The following thesis is a compilation of literature reviewed to formulate background research for a pilot study. The literature reviewed pertains to the potential benefit of added case management for patients with diabetes mellitus type 2. A proposed pilot study, based off of this research, aims to identify topics, tools, facts, and changes that can be made to case management to improve not just the biological aspects, like HbA1c levels, but the mental and emotional as well to allow for whole person care.

Purpose: The purpose of this retrospective, mixed methods study is to examine the relationship between participation in an interdisciplinary diabetes self-management education (DSME) program at an urban primary health care center and patients’ perceived knowledge and skills, as well as clinical markers, on four cohorts of patients over a two-year period. Methods: Participants, mainly African-American females, responded to survey questions including self-care behaviors, perceived knowledge, and self-efficacy. The researchers also reviewed the participants’ clinical records for glycosylated hemoglobin (HbA 1c) and body mass index (BMI) data and compared these to similar patients in the health center who had …
