Contingency Management For Smoking Cessation In Pregnancy, Jacob Miller, Brady Reynolds, Courtney Crain, Hisham Keblawi, Lynne J. Goebel

This pilot study examined the feasibility of applying Contingency Management (CM), where the woman receives financial compensation for abstinence from smoking. Eleven subjects were recruited from Marshall’s OB clinic for the program. CO levels were measured daily using online video recording. Quit rates were determined at the end of pregnancy. Birth outcomes were measured at delivery. 21 pregnant smokers from Marshall’s OB clinic were used as the control group. The results demonstrated a trend toward improved rates of smoking abstinence in CM participants as compared with the control group. Our study also showed a trend towards improved birth outcomes in …

Numerous studies have examined how both negative and positive maternal exposures (environmental contaminants, nutrition, exercise, etc.) impact offspring risk for age-associated diseases such as obesity, type 2 diabetes, hypertension, and others. The purpose of this study was to introduce the foreskin as a novel model to examine developmental programming in human neonates, particularly in regard to adipogenesis and insulin receptor signaling, major contributors to age-associated diseases such as obesity and diabetes. Neonatal foreskin was collected following circumcision and primary dermal fibroblasts were isolated to perform adipocyte differentiation and insulin stimulation experiments. Through our work in youth advocacy as, respectively, legal and public health professionals, we are all too aware of the high levels of health care fragmentation experienced during pregnancy and postpartum by poor, young mothers of color. Meredith Matone’s research highlights the heightened risk of fragmentation for girls with histories of child welfare involvement. For example, she found that 66.7% of young mothers who had resided in out-of-home placements and who had taken antipsychotic medication prior to becoming pregnant failed to fill prescriptions for antipsychotics in their first postpartum year. This study investigates the maternal birth experience through a cross-cultural lens. While the field of medical anthropology has researched birth practices of many cultures, few cross-cultural studies have been performed and no recent studies have suggested a transition in birthing. Ethnographic interviews with women and practitioners in Yucatán, Mexico and with women in Kentucky, United States allowed for a better understanding of the respective birthing environments. Grounded theory was then employed to develop a birth transition theory explaining changes occurring when society transitions from traditional birth practitioners to allopathic birth practitioners. In this project, I discuss the impact of social, medical, and political factors on the reproductive healthcare of adolescents in Cachoeira, Bahia, Brazil. I collected qualitative data through formal interviews with public health professionals in the Sistema Única de Saúde (SUS), the Brazilian public health system, and through observations of clinic and home visits. Due to ethical concerns, I did not conduct formal interviews with the patients, were not of legal age. I focused on prevention of pregnancy and STI transmission of adolescents as metrics with which to measure sexual health. I gathered information with the intention to understand SUS’ …

A woman’s first pregnancy can be both emotionally exciting and daunting. There are many changes to make, but there is little emotional support to adjust to the role, the focus being on the physical process which is most often managed medically (Spear, 2008; Zasloff, Schytt, & Waldenström, 2007) though warnings about what could occur are not routinely told (Kaitz, 2007, pp. 720-721). Expectant parents who have been exposed to psychosocial risk encounter deleterious psychological (Ashley et al., 2016), and physiological (V. H. Pereira, Campos, & Sousa, 2017) effects. This not only affects the parent-to-be, but also may affect the developing fetus (E. P. Davis et al., 2011) and is linked to poorer infant development (Lefmann & Combs-Orme, 2014). However, not all risk-exposed individuals experience this, many are resilient and still thrive in the face of adversity. Understanding potential risk and resiliency factors in expectant parents is advantageous, so tailored interventions can be devised to improve outcomes.
