Perinatal substance use (PSU) is a serious and growing public health concern. It is associated with a variety of adverse health outcomes for both mother and child and has been shown to negatively impact the parent-child relationship. Despite the growing prevalence of PSU, there are notable deficits in provider knowledge regarding, and comfortability with, PSU. Moreover, providers report feelings of judgment, resentment, fear, and hesitancy related to their work with women with PSU. Subsequently, women with PSU struggle to find appropriate, compassionate, and effective treatment for their substance misuse. This dissertation is a collection of four studies which collectively explore a hypothesized construct of ‘moral evaluation of patient behaviors’ (MEPB) as a driver of health professionals’ readiness to interact humanistically with their patients. In these studies, ‘humanistic interactions’ refer to the non-technical, intangible skills and factors of clinical competence; the factors specifically explored in these studies were compassion toward patients, self-efficacy for treating patients, and optimism toward patient treatment. However, only 5-10% of individuals with substance use problems co-occurring with mental health problems are correctly identified. The purpose of this research was to identify predictors of relapse using three different instruments of varying complexity: the Patient Health Questionnaire-9 (PHQ-9), the Modified Mini Screen (MMS), and the Millon Clinical Multiaxial Inventory-III (MCMI-III). Data was used from the first three waves of the National Longitudinal Study of Adolescent Health survey. This study found that being a victim of childhood physical abuse can lead to abuse of alcohol later in life. In 2011, the U.S. reported 3 million child maltreatment cases, an uncomfortably high but recurring figure each year. Research shows exposure to early life stress (ELS) increases an individual’s susceptibility to substance abuse, specifically of nicotine, alcohol, and cocaine. Increased susceptibility may result from dysregulation of the HPA axis sustaining activation into adulthood after ELS. Hyperactivation of the HPA axis significantly reduces hippocampal BDNF, a neurotrophin involved in neuronal growth and plasticity. Reduced hippocampal BDNF may be a factor in substance abuse vulnerability. Additionally, research shows exercise protects hippocampal BDNF from stress induced down-regulation. The purpose of the literature review is to identify consistent patterns regarding adolescent use and abuse of chemical substances, especially alcohol and marijuana. Acute physical, cognitive and social effects of alcohol and marijuana use are outlined, and environmental and personal determinants of drug use and abuse are examined. Methods of prevention and intervention are discussed and, from the research findings, a model group counseling program designed for the school setting is proposed.
