Background. Hepatitis C virus (HCV) infection engenders substantial metabolic changes. These changes are altered when the virus is cleared after successful treatment. We measured these metabolic alterations that occur after HCV cure; further, we assessed whether these alterations differed in subgroups defined by patients’ characteristics.

Spain has been the global leader in organ donation and transplantation since 1992, an achievement that has become a source of national pride, in a country where national symbols are heavily contested. In this dissertation I examine the changing meanings that organ donation and transplantation have acquired in contemporary Spain, focusing specifically on their implications for different aspects of Spanish nationalism. Despite decades of concerted efforts to prevent their spread, chlamydia andgonorrhea remain two of the most prevalent sexually transmitted infections in the United States (U.S.) — exacting a high toll in terms of human health and healthcare expenditure. Though easily cured with antibiotics, both infections may lead to damaging secondary health conditions – known as sequelae – if untreated, including infertility among females. Compulsive exercise is poorly understood and difficult to treat. Overlap between definitions of compulsive exercise, eating disorders and OCD suggests that conceptualizations of these disorders may include shared underlying mechanisms, including intolerance of uncertainty. Although intolerance of uncertainty is traditionally conceptualized as a non-specific construct representing an individual’s dispositional distress in the face of general uncertainty, identifying specific forms of intolerance of uncertainty may further assist in understanding the specific factors that contribute to the development and maintenance of problematic behaviors, including compulsive exercise. Pregnancy-related maternal morbidity and mortality disproportionately affect Black women more than their White counterparts. Black pregnant women are more likely to omit or engage in prenatal care late than White women. Social media is an essential source of pregnancy-related information and shows it effectively improves pregnancy knowledge. Greater than 80% of Black women own a mobile device, and some data demonstrate that Black women use social media for pregnancy-related information. Understanding digital health literacy (DHL) in this unique population and subsequent prevention behaviors in a digitally connected population during a public health crisis is crucial to prepare for future pandemics. This study explores the role of medical discourse in the Iranian formal public sphere. It examines how an epidemiological wave of HIV/AIDS—known as "the third wave"—highlighted nontraditional sexual behaviors in public and enabled a shift in policy and discourse by the Islamic Republic State. Through analyzing published content on HIV/AIDS from five major Iranian newspapers between 2009 and 2013, this study identified four competing narratives of the third wave which coexist and have a dynamic relationship with one another. The COVID-19 pandemic has disrupted people's lives and food systems in the United States and worldwide, resulting in impacts on different aspects of food security. To understand how this has affected households in New York’s Capital Region, we conducted two concurrent anonymous online surveys between October 2020 and February 2021. Combined, 1049 people responded to these surveys.

We report on respondents' food security experiences since the COVID-19 pandemic started in New York State (March 7, 2020) compared to the prior year. We further explore the differential impacts of the pandemic on food security experiences by race and ethnicity, household income, …

In this dissertation, I aim to forecast high utilizers of emergency care and inpatient Medicare services (i.e., healthcare visits). Through a literature review, I demonstrate that accurate and reliable prediction of these future high utilizers will not only reduce healthcare costs but will also improve the overall quality of healthcare for patients. By identifying this population at risk before manifestation, I propose that there is still time to reverse undesirable healthcare trajectories (i.e., individuals whose clinical risk increases an excessive healthcare and treatment burden) through timely attention and proper care coordination. The opioid crisis has highlighted the importance of improving patients’ access to behavioral treatments for chronic pain and addiction. What is not known is if patients are interested in receiving these treatments. In this cross-sectional study, over 1000 participants with chronic pain were surveyed using an anonymous online questionnaire on Amazon Mechanical Turk (Mturk) to investigate participants’ use of and interest in pharmacological and behavioral treatments for chronic pain and addiction. Participants also indicated whether their doctor had recommended these treatments. The majority of participants reported using medication for their pain (83.19%) and that their doctor recommended medication (85.05%), whereas …

Specifically, this study investigated the distinct and overlapping impacts of (a) communication based in the Common Sense Self-Regulation Model (CSM; Leventhal et al., 1980), and (b) the interpersonal relationship between the patient and the dental health professional provider on patients’ oral hygiene quality and habits. Latinx men who have sex with men (LMSM) are at significantly greater risk for HIV infection than their non-Latinx peers. One important way of helping prevent the spread of HIV is exposing vulnerable populations to HIV prevention programming. Little is known about what factors predict exposure to HIV prevention programming for LMSM. Given extant links between traditional masculine gender role norms and health outcomes, this study sought to examine possible relations between constructs of machismo, caballerismo, and heterosexual self-presentation, along with demographic covariates, and exposure to HIV prevention programming among LMSM. In this thesis, I take up the question of how the Medicare penalties introduced by the Hospital Readmissions Reduction program (HRRP) impacted the US health care system. The demand side of US health care system is highly fragmented where hospitals and other providers have to serve very different kinds of patients from different payers. Since the majority of a hospital's income is drawn from Medicare, a sharp reduction in its reimbursements is bound to have an effect on its health care delivery to all patients.

Despite growing recognition that misdiagnoses of child abuse can lead to wrongful convictions, little empirical work has examined how the medical community may contribute to these errors. Previous research has documented the existence and content of stereotypes that associate race with child abuse. The current study examines whether emergency medical professionals rely on this stereotype to fill in gaps in ambiguous cases involving Black children, thereby increasing the potential for misdiagnoses of child abuse. Specifically, we tested whether the race-abuse stereotype led participants to attend to more abuse-related details than infection-related details when an infant patient was Black versus White. …

This experiment was designed after the release of the Netflix show called the “The Staircase.” A true documentary, a woman’s fall down the staircase results in her death, but the investigators question whether it was an accident or murder. The woman’s husband was initially accused of murder, but after a lengthy trial and the review of additional evidence, it was ruled an accident. In the autopsy results, there was evidence of blunt force trauma (Figure 1) inconsistent with an accidental fall. Therefore this research asks what is the difference in trauma from an accidental fall versus a intentional, assisted descent?

This literature review attempts to provide a more complete understanding of the increasing opioid concern and its detrimental effects from withdrawal in incarcerated individuals. Opioid-related fatal overdoses will significantly decrease with the implementation of opioid treatment in jails and prisons nationwide. The incarcerated populations that are typically affected by Opioid Use Disorder (OUD) include about 8.5% of Hispanics and Whites, and about 7.4% of Blacks. One solution involves Medication-Assisted Treatment (MAT) with the use of opioid agonists, such as methadone and buprenorphine-naloxone. These medications offer a slow release of dopamine and can reduce the patient’s opioid withdrawal effects. However, fewer …

Competency to stand trial is a concept of jurisprudence preventing the adjudication of individuals who lack the capacity to understand the proceedings against them and which prevents them from assisting in their own defense because of mental disease or “defects”. In the criminal justice system, defendants found incompetent to stand trial are remanded to the care of the mental health system for a period of time to restore their competence. The determination of competency to stand trial is by far the most common psychiatric evaluation requested by criminal courts, and is especially critical for defendants with developmental and intellectual disabilities.

In 2008, New York’s health commissioner asked LHDs and nonprofit hospitals to collaborate with community partners to conduct community health needs assessments, identify two or three health priorities to focus on, and implement evidence-based strategies to address them. The actions taken as part of the New York State Prevention Agenda mirrored the IRS rules and PHAB recommendations that followed. The dearth of published studies on public health-hospital collaborations and the momentum to understand how communities in New York have responded to the IRS rules and PHAB standards are the impetus for this research.
