Shoulder injuries occurred frequently in elite baseball players. They are generally evaluated by questionnaire and physical examination and lacking of objective study such as a high-resolution ultrasonography especially in Taiwan. This study attempted to investigate the status of shoulder injuries in Taiwan elite baseball players by questionnaire and physical examination and defined the role of ultrasonographic examination in this type of injury.Twenty four elite college baseball players were recruited in this study. To determine whether women participated in sport, even these young nulliparous female athletes, are more prone to have urinary incontinence in Taiwan. Four hundred and forty- six young female students were included in this study. All of them were nulliparous. Our subjects consisted of 260 varsity athletes, 106 athletes of high school, and 80 varsity non-athletes. They were requested to complete questionnaires on the type of major sport, intensity of training, menstrual history, sport injury, and urinary incontinence. The purpose of the study was to investigate the interaction between pain, pain behavior, anxiety, and muscle tension during electrodiagnosis, and to examine the anxiolytic and pain-relieving effects of traditional Chinese music. Two measuring tools, including electrodiagnosis pain behavior checklist (EPBC) and electromyography muscle tension scale (EMTS) were developed to objectively assess the subject's pain and muscle tension, respectively. Sixty-one patients were included in this study and were randomly divided into background music group and control group. The purpose of the study was to measure the peak pressure under the metatarsal heads (MTH) of young men and women in walking with high resolution pressure system and to determine their reliability. The peak pressure of sixteen health men and women, aged from twenty-one to thirty-five years-old, were measured at the first, second and third metatarsal head by high resolution pressure system for ten repetitions using first-step method. Spinal cord infarction is a rare disease. It is even more uncommon that spinal cord infarction occurs in a patient without any cardiovascular disease. Anterior spinal artery syndrome is much more common than the posterior spinal artery syndrome due to the difference in collateral supply. We present two patients with spinal cord infarction. In both patients, the clinical picture was stereotyped: sudden onset of weakness of limbs, sensory loss below the level of infarction and sphincter dysfunction. Treatment for the two patients was supportive and they had a good recovery over a period of weeks.

The clinical appearance of aortic dissection is characterized by the sudden, acute chest or abdominal pain, accompanied by coldness in the limbs and sweating-like symptoms of shock. Absent pulse or asymmetric pulse was observed in some patients. If the feeding arteries of the specific organs are located in the false lumen, this may lead to sudden loss of blood supply to the tissue e.g. spinal cord, resulting in cord damage. OBJECTIVE: To determine the effect of noncontact normothermic wound therapy (NNWT) versus standard wound care on chronic full-thickness pressure ulcers.

DESIGN: Prospective, randomized, controlled trial

SETTING: Veterans administration medical center and 7 long-term-care facilities

PATIENTS: 40 inpatients with 43 Stage III and IV pressure ulcers

INTERVENTIONS: A sterile noncontact wound dressing was applied to 21 wounds for 24 hours per day, 7 days per week. Each day after the wound was irrigated and the noncontact dressing was changed, a heating element in the dressing was activated for 3 1-hour periods for 12 weeks or until wound closure. The purpose of this study was to survey the handedness performance in the children with cerebral palsy (CP). One hundred and eighty two cerebral palsied children were referred for study by the primary schools in the Taipei city. Handedness Inventory with two factors was developed and sent to their parents. Response to the survey was 116 (116/182 = 64%) with 101 of them being analyzed in this study. The mean handedness quotient was 34.39. It became 53.33, 46.59, and .32 in diplegias, quadriplegias and hemiplegias, respectively. One-way analysis of variance revealed statistically significant differences among these three types. A survey of effectiveness of home based team rehabilitation at Tai-Chung City was done form January to December 2001. Nineteen subjects (76.0% of total samples) were adopted for analysis. The rehabilitation programs included intervention once per week for six months and three times evaluation of ADL function. The results showed that 1) Both the first three months home based team rehabilitation and the following three months intervention had effectiveness on Barthel ADL index; the scores improved from 42.11±27.90 to 46.32±29.67 and 51.58±30.42 after 3 months and 6 months intervention; 2) Severity had inverse correlation to the score of Barthel ADL …

Primary antiphospholipid syndrome is a rare disease of vasculopathy, which is usually found in the stroke partient with onset during childhood and young adulthood. We report a female patient who had a history of multiple spontaneous abortions in the past. However, the diagnosis was not made until a cerebrovascular accident happened to her. After then, she started to receive anticoagulant therapy and rehabilitation program. No further cerebrovascular accident occurred in the following 22 months. By this report, we remind that a female patient with history of frequent abortion needs to be investigated carefully to search the possibility of antiphospholipid syndrome.

Bilateral peripheral facial palsy (BPFP) is a rare disease. It occurs in less than one percent of patients with facial palsy. The average annual incidence rate is 0.13 to 0.2 persons per million. The etiologies of BPFP differ a little from those of unilateral peripheral facial palsy. The diagnosis depends on a series of complete examinations. We reported two cases with BPFP. Extracorporeal shock wave therapy (ESWT) has shown success in the alleviation of painful heels and other orthopedic conditions such as tennis elbow, calcifying tendinopathy of the shoulder, and nonunion fractures of the long bones. Diffuse idiopathic skeletal hyperostosis (DISH) is a common disorder in middle-aged and elderly patients but often ignored. It is characterized by calcification and ossification of the spinal ligaments, as well as entheseal ossification within extraspinal sites. The etiology of DISH is still unknown. It can result in chronic pain and stiffness and occasionally severe disability. It often mimiks with spondylosis and ankylosing spondylitis. Objective: To determine the effects of Vicoprofen® and ibuprofen on aerobic performance, agility, and pain after exercise-induced muscle damage. Design: Double-blind randomized, placebo-controlled, repeated-dose clinical trial. Setting: Human-performance and sports-medicine laboratory. Participants: 36 healthy men. Methods and Measures: Baseline testing was performed, 72 hours after which subjects performed eccentric exercise to induce muscle damage. They were evaluated for pain 24 hours postdamage and placed randomly into 3 groups: Vicoprofen (VIC), ibuprofen, or placebo (P). Postdamage testing was performed every day for 5 days. This case study describes the process of examination, re-examination, and intervention for a collegiate runner with mechanical thoracic pain preventing athletic participation and limiting daily function. Unimpaired function fully returned in less than 3 weeks with biweekly sessions to re-establish normal and painfree thoracic mechanics via postural hygiene, exercise, mobilization, and manipulation. The outcome of this case study supports the original hypothesis that the pattern of impairments was in fact responsible for the functional limitations and disability in this athlete. The clinical presentation of psoas abscess is variable and nonspecific. The triad of fever, flank pain, and limitation of hip movement, which is specific for psoas abscess, is present in only less than 30% of the cases. Therefore, delayed diagnosis or misdiagnosis, resulting in the high mortality and morbidity, is not uncommon. A primary psoas abscess has no obvious focus of infection. Urinary retention is an uncommon complication of herpes zoster infection. It may happen secondary to a neurogenic bladder when the sacral dorsal root ganglia are involved. As a result, painful vesicular eruptions spreading over sacral dermatomes, such as buttock and perineum, may develop. Bladder and/or bowel dysfunction may occur as well, including urinary retention, constipation, and/or incontinence. The herpes zoster lesions usually have an unilateral dermatomal distribution. A 55-year-male patient with herpes zoster infection involving bilateral S2-S4 dermatomes is demonstrated. Acute urinary retention occurred 2 days before skin vesicular eruptions. The purpose of the study was to reveal any abnormal distribution of the static and dynamic plantar pressure in the diabetic patients with advanced neuropathy. Sixteen diabetic patients with advanced neuropathy (Group N) and 16 diabetic patients without advanced neuropathy (Group D) were recruited. Advanced neuropathy was determined by inability to perceive the 5.07 Semmes-Weinstein monofilament at one or more sites in each foot. This 25 years old female was found comatous for 24 hours after she injected heroin for attempting suicide. She had no history of drug abuse and no history of inhalation or injection of heroin. Crossed aphasia is defined as aphasia due to right hemisphere lesion in dextrals. It is a rare syndrome. It was reported first time in 1899, then it elicited much discussion and studies. Spinal epidural abscess is an uncommon disease, and is rarely reported as a complication of acupuncture. We present two cases of spinal epidural abscess which developed after acupuncture treatment. Case 1 was 61 years old male who suffered from progressive pain and weakness over right upper arm after cervical manipulation and acupuncture. He became qudriplegia with urine retention 6 days later. Cervical MRI showed C3-6 epidural abscess. After operation and antibiotics treatment, he recovered to qudriparesis and could walk with the spastic gait 9 months later. This article will provide an overview of the problem-based learning process, and an illustration of how problem-based learning can facilitate development of critical appraisal necessary for evidence-based practice. It will trace the development of problem-based learning as an educational methodology for entry-level health professional education.
