What is already known on this topic? Although the implementation strategies and effectiveness of quality improvement (QI) activities have been examined extensively for many industries, including the health care sector, very few studies have focused on QI activities in the public health context.

What is added by this report? The study results indicated that Nebraska’s LHDs still face significant barriers for QI implementation, including low capacity, knowledge gaps, inadequate resources, and low institutional QI maturity.

Frontiers in Public Health Services and Systems Research

There is a gap in knowledge about how to best organize and administer practice in local health departments to implement sustained evidence-based policies, programs, and interventions. This report identifies administrative and management evidence-based practices to inform ongoing initiatives in local public health system quality improvement, accreditation processes, and performance. The article presents administrative elements in workforce development, leadership, organizational climate and culture, relationships and partnerships, and financial processes that local health departments can address at modest cost within a few years or less. Local public health systems can further identify, implement and evaluate evidence-based administrative practices.

Objective: This article describes the evaluation of a new model of partnership care using an audit cycle.

Results: Statistically significant improvements in foot examination, body mass index, urine albumin creatinine ratio, total cholesterol, triglycerides and visual acuity measurements were observed. Significant increases in the proportion of patients achieving cholesterol and triglycerides therapeutic targets occurred. Most other outcome indicators demonstrated a nonsignificant improvement, which may be due to the short time interval in the audit for potential change.

This dissertation, comprised of three publishable manuscripts, addresses various identified gaps in the literature focused on public health systems and services research. Public health systems and services research includes studies that examine the organization, financing and delivery of public health services in communities, and the impact of these services on public health.
