Priority Setting In Indigenous Health: Why We Need An Explicit Decision Making Approach, Michael E. Otim, Ranmalie Jayasinha, Margaret Kelaher, Edward Shane Houston, Ian P. Anderson, Stephen Jan
Michael E Otim
Indigenous Australians have significantly poorer health outcomes than the non-Indigenous population worldwide. The Australian government has increased its investment in Indigenous health through the "Closing the Health Gap" initiative. Deciding where to invest scarce resources so as to maximize health outcomes for Indigenous peoples may require improved priority setting processes. Current government practice involves a mix of implicit and explicit processes to varying degrees at the macro and meso decision making levels. In this article, we argue that explicit priority setting should be emphasized in Indigenous health, as it can ensure that the decision making process is accountable, systematic, and …
Defining The Content And Delivery Of An Intervention To Change Adherence To Treatment In Bonchiectasis (Can-Be): A Qualitative Approach Incorporating The Theoretical Domains Framework, Behavioural Change Techniques And Stakeholder Expert Panels, Amanda Mccullough, Cristin Ryan, Brenda O'Neill, Judy Bradley, J Stuart Elborn, Carmel Hughes

Background: Low patient adherence to treatment is associated with poorer health outcomes in bronchiectasis. We sought to use the Theoretical Domains Framework (TDF) (a framework derived from 33 psychological theories) and behavioural change techniques (BCTs) to define the content of an intervention to change patients’ adherence in bronchiectasis (Stage 1 and 2) and stakeholder expert panels to define its delivery (Stage 3).

Parents’ Beliefs And Knowledge About The Management Of Acute Otitis Media: A Qualitative Study, Malene Hansen, Janine Howlett, Chris Del Mar, Tammy Hoffmann
Christopher Del Mar
Acute otitis media is a common reason for antibiotic prescribing, despite strong evidence that antibiotics provide minimal benefit. Studies have demonstrated that patients’ (or parents’) expectations of antibiotics often influence general practitioners’ (GPs) decision to prescribe antibiotics, but few have explored parents’ expectations of the management of infections in children, or which factors influence the development of these expectations. This study aimed to explore parents’ knowledge and beliefs about the management of acute otitis media in children.

Today’s medical students (tomorrow’s doctors) will be entering a world of conflict, war and regular outbreaks of infectious diseases. Despite numerous international declarations and treaties protecting human rights, the last few decades has been fraught with reports of ‘‘lapses’’ in medical professionalism involving torture and force-feeding of detainees (e.g. captured during the War on Terror) and health care professionals refusing to treat infected patients (e.g. HIV and Ebola). This paper provides some historical background to the changing status of a physician’s duty to treat and how medical practitioners came to be involved in the inhumane treatment of detainees during the …

The importance of regular physical activity to overall health has been well established, yet U.S. adults are leading increasingly sedentary lives. Research suggests that lowering perceived barriers to physical activity is a critical part of interventions that encourage physical activity. In this paper, we describe the top five barriers—two of which have not been reported as principle barriers in the literature—that emerged from our systematic qualitative coding of an online forum used to support a three month healthy lifestyle intervention. Based on our analysis, we identify design considerations for technologies that encourage and support physical activity. Understanding the needs of …

Public Health Marketing: Is It Good And Is It Good For Everyone?, Stephen Holden, Damian Cox

We define public health marketing broadly as the use of marketing tools (segmentation, targeting, positioning, and the four Ps) to encourage behaviour change that will deliver the social good defined as public health. We explore the ethical challenges and risks that confront public health and social marketers. In particular, we note that public health marketers with a self-defined goal of delivering a social good face two major ethical challenges: the first is establishing the ethicality of the social good itself; the second is distributing the social good in an ethically defensible way. In particular, we draw attention to the central …

Dr Brendon P Hyndman
Background: An emerging public health strategy is to enhance children’s opportunities to be physically active during school break periods. The aim of this study was to evaluate the effects of the Lunchtime Enjoyment Activity and Play (LEAP) school playground intervention on primary school children’s quality of life (QOL), enjoyment and participation in physical activity (PA). Methods: This study consisted of a movable/recycled materials intervention that included baseline, a 7-week post-test and an 8-month follow-up data collection phase. Children within an intervention school (n = 123) and a matched control school (n = 152) aged 5-to-12-years-old were recruited for the study. …

The aging process is commonly associated with declines in health, cognitive function and well-being. However, lifestyle factors like diet, alcohol consumption, smoking and physical activity were repeatedly highlighted as predictors of a healthy aging process. However, recent research has shown that physical activity is the strongest predictor of health in older adults. Recent studies have confirmed the strong effect of physical activity on cardiovascular, metabolic, musculoskeletal and mental health in this age group, while the World Health Organization and other bodies have published physical activity guidelines. Despite the overwhelming evidence of the positive effects of physical activity older adults around …

Human Development And Resources Use In The Coastal Zone: Influences On Human Health, Robert E. Bowen, Anamarija Frankic, Mary E. Davis
Robert E. Bowen
Coastal watersheds and nearshore marine areas are the most valuable and dynamic places on Earth. Human population growth is great in these regions, which are home to some of the most sensitive habitats in the world. Coastal areas provide more than half of the overall service value derived from the global environment (Costanza et al., 1997). Natural (e.g., hurricanes and tsunamis) and human pressures on this environment require it to constantly adjust. More than any other area, the global coast has defined the progress of human culture and continues to be a singular influence in how humans connect to the …
Implementing A Mobile Wireless Environment In A Hospital Ward: Encouraging Adoption By Nursing, Julie Fisher, Linda Dawson, Stephen Weeding, Liza Heslop
Implementing A Mobile Wireless Environment In A Hospital Ward: Encouraging Adoption By Nursing, Julie Fisher, Linda Dawson, Stephen Weeding, Liza Heslop
Associate Professor Linda Dawson
Sophisticated technology is commonplace in most hospitals and increasingly mobile devices are being used in hospitals by clinical staff. Although the growth in mobile device usage in hospitals has the potential to contribute to better health and medical services delivery, nurses and doctors are still very reliant on paper-based information. Much of the research reported to date has focused on technical and design issues around mobile devices. Research that has focused on mobile device use in practice has tended to be from the perspective of doctors. This paper describes research which investigated key issues that arose as a result of …

Management Of Patients With Cognitive Impairment After Stroke: A Survey Of Australian Occupational Therapists, Chia-Lin Koh, Tammy Hoffmann, Sally Bennett, Kryss Mckenna
Tammy Hoffmann
Background/aim: Cognitive impairment is a common and often debilitating consequence of stroke. The current practice patterns of Australian occupational therapists who work in this area are not clearly known. The aim of this study was to investigate the theoretical approaches, assessments, interventions and research evidence used by Australian occupational therapists who work with patients who have cognitive impairment poststroke.
Methods: A self-administered, purpose-designed online survey was used.
Results: Survey responses were received from 102 occupational therapists. The client-centred approach was the most commonly used theoretical approach, with 81.3% and 72% using it often or all of the time with inpatients …

The Irrational Woman: Informed Consent And Abortion Decision-Making, Maya Manian

In Gonzales v. Carhart, the Supreme Court upheld a federal ban on a type of second-trimester abortion that many physicians believe is safer for their patients. Carhart presented a watershed moment in abortion law, because it marks the Supreme Court’s first use of the anti-abortion movement’s “woman-protective” rationale to uphold a ban on abortion and the first time since Roe v. Wade that the Court denied women a health exception to an abortion restriction. The woman-protective rationale asserts that banning abortion promotes women’s mental health. According to Carhart, the State should make the final decisions about pregnant women’s healthcare, because …

Health Hazard Manual For Firefighters, Nellie J. Brown
Health Hazard Manual For Firefighters, Nellie J. Brown
Nellie J. Brown, MS., C.I.H.
[Excerpt] Firefighters, as well as victims, can be exposed to a variety of toxic substances during a fire. Some of these toxicants are particularly insidious because they are produced by thermal decomposition before smoke makes a fire evident.
Health Hazards Manual For Autobody Shop Workers, Nellie J. Brown
Health Hazards Manual For Autobody Shop Workers, Nellie J. Brown
Nellie J. Brown, MS., C.I.H.
[Excerpt] We will look at the principal occupational health hazards and exposures themselves and some of the related issues. We will look closely at the chemical composition of autobody shop products to see what components appear to be particularly hazardous, how you are exposed to them, and what you can do to minimize exposure. The health effects discussed for these products are based upon the exposure of the professional, not the consumer. This manual deals with chemical exposure only; other issues of interest to the autobody shop worker may include occupational exposure to noise and neuromuscular problems such as Raynaud’s …
