Objective: To evaluate the current literature evidence for the effects of either instructed or spontaneous pushing on perineal laceration incidence during delivery and the duration of second stage of labor.

Background: Lacerations (tears) of the perineum are common among women during delivery, increasing pain, infection risk, and other problems for women. Furthermore, prolonged second stage of labor has been shown a risk factor for lacerations. Pushing methods could have an effect on the incidence of lacerations and duration of second stage of labor.

Thirty-five percent of mortalities in children under the age of five can be attributed to malnutrition (World Health Organization (WHO), n.d.). Nearly 24 percent of children less than five from 2008-2012 in the Central African Republic were moderately or severely underweight and nearly eight percent suffered from severe or moderate wasting (The United Nations International Children’s Emergency Fund, 2013). Intrauterine exposure to nicotine is one of the largest modifiable risk factors for infant morbidity and mortality. Previous progress toward decreasing the number impacted by intrauterine nicotine exposure has recently plateaued. While guidelines for assisting pregnant women to stop smoking are available, few providers have a formalized protocol in place. This project will evaluate most current research evidence regarding smoking cessation during pregnancy in order to develop a formalized protocol for implementation in an obstetric clinic in Southwest Ohio. PICOT: In pregnant women does the use of nicotine replacement therapy compared to smoking during pregnancy reduce the risk of future childhood health concerns?

Methods: A search was conducted on healthcare literature databases (Alt HealthWatch, AMED, CINAHL, and Medline).The initial search yielded 940 results related to the topic. Initial review narrowed the search to 25 articles. Articles were excluded if published before 2008. After reviewing the full articles and evaluating effectiveness of the studies, 12 studies met the criteria. These 12 articles focused on the effects of smoking and childhood outcomes, NRT, and success of NRT.
