Turnover in the child welfare workforce remains a problem with detrimental consequences. While a robust body of literature has explored the influence of job factors on employee retention, and the presence of secondary traumatic stress and other related experiences in this population, little is known about the impact of such factors on the physical health of the practitioner. This manuscript is a first step in documenting the relationship between worker characteristics, perceptions of their job, and their self-reported health status. Background: Research has shown that adverse childhood experiences are strongly linked with health outcomes over the life course, and that child maltreatment – generally defined as physical abuse, sexual abuse, emotional maltreatment and neglect – can have an immediate, negative impact on child health and development and a longer-term impact on adolescent and adult health, including the leading causes of morbidity and mortality in the U.S. With such significant consequences, the accurate identification of child maltreatment is critical.

Background: Youth in protective custody (i.e., foster care) are at higher risk for poorer physical and mental health outcomes compared with those who are not. These differences may be due in part to the lack of research on the population to create evidence-based recommendations for health care delivery. A potential contributor to this lack of research is difficulties in obtaining informed consent for empirical studies in this population. The objective of this study was to describe the approaches to obtaining informed consent in minimal risk studies of foster youth and provide recommendations for future requirements.

Opioid use disorder impedes dependent parents' abilities to care for their children. In turn, children may languish in unpredictability and persistent chaos. Societal responses to these children are often guided by a belief that unless the drug dependent parent receives treatment, there is little help for the child. While a preponderance of the drug dependence research is adult-centric, a significant body of research demonstrates the importance of not only addressing the immediate well being of the children of drug dependent caregivers but preventing the continuing cycle of drug dependence. Through our work in youth advocacy as, respectively, legal and public health professionals, we are all too aware of the high levels of health care fragmentation experienced during pregnancy and postpartum by poor, young mothers of color. Meredith Matone’s research highlights the heightened risk of fragmentation for girls with histories of child welfare involvement. For example, she found that 66.7% of young mothers who had resided in out-of-home placements and who had taken antipsychotic medication prior to becoming pregnant failed to fill prescriptions for antipsychotics in their first postpartum year. Given the cost of long-term foster care placement in both human and economic terms, few studies have specifically explored if any factors help to predict why this vulnerable population spends significantly more time in foster care. The overarching goal of this exploratory study was to use binary logistic regression to investigate whether any child demographic or environmental characteristics predicted the discharge of a child placed in Kentucky's foster care system for child sexual abuse. Teens in foster care give birth at over twice the rate of other teens. Unique challenges exist for these vulnerable teens and babies, yet research on such populations, particularly within the systems that serve them, is limited. A demonstration project at Inwood House, a residential foster care agency in New York City, from 2000 to 2005, at the same time that the Administration for Children's Services was exploring policy and practice changes for this population, is described. Increasingly, scholars and policymakers are calling for programs that take a preventive approach to child abuse and neglect, rather than our current tendency to respond only after a crisis. There are significant social and economic arguments supporting this shift. The Nurse-Family Partnership, developed by David Olds and discussed in this symposium, illustrates how specific investments in family functioning can lower rates of child abuse and neglect, leading to a host of positive outcomes for children and society, from greater educational attainment to less involvement in the criminal justice system. This article reviews the importance of regional initiatives in the context of global efforts to achieve the Millennium Development Goal 4 and 5 and describes the action-oriented multi-country healthcare professional association (HCPA) workshops organized by the Partnership for Maternal, Newborn and Child Health. The South Asian HCPA workshop served as a catalyst for strengthening the ability of HCPAs in South Asian countries to organize and coordinate their activities effectively, play a larger role in national planning, and collaborate with other key stakeholders in maternal, newborn and child health.

Fetal Alcohol Spectrum Disorder (FASD) is a serious social and health problem for the child welfare, health and education systems in North America and other parts of the world. This article describes the population of children in care of the child welfare system in Manitoba. Also this article will highlight the relevance of these research findings to aboriginal populations in Canada and its implications for international aboriginal/ indigenous groups. Finally, the implications for policy, practice are discussed and the article puts forward some directions for further research.

Future Orientation Of Adolescents In Foster Care: Relationship To Trauma, Mental Health, And Hiv Risk Behaviors, Peter Cabrera, Wendy Auslander, Michael Polgar

Future orientation has been found to protect against risk behaviors in adolescents from diverse backgrounds. However, no studies have specifically examined future orientation as a potential protective factor against HIV risk behaviors in foster care adolescents. In this study, 343 foster care adolescents were interviewed about their future orientation, mental health, trauma histories, and cognitions related to HIV risk behaviors. Results indicated variability in future orientation, but there were no significant differences by race, gender, and age. Future orientation was significantly associated with mental health, trauma, HIV-related knowledge, attitudes, behavioral intentions, and number of sexual intercourse partners. In this essay the author comes to the following conclusions based upon a civil liberties analysis. First, surrogacy arrangements cannot be prohibited or criminalized. Second, the state cannot ban the exchange of money for surrogacy services, provided the money is paid for conception, gestation, and birth. Money, however, cannot be paid on condition that the gestational mother waive her parental rights over the child. Third, contractual provisions that require the gestational mother to waive her parental rights or her rights to privacy and autonomy are void and unenforceable.
