Growth Hormone Deficiency: When Is Mri Imaging Indicated?, Haya Azouz, Kurt Tech, M Bassem Dekelbab

Conference Presentation Abstracts

Objectives The diagnosis of Growth Hormone Deficiency (GHD) is made with a combination of clinical assessment, measurement of insulin-like growth factor 1 (IGF-1) and insulin-like growth factor binding protein-3 (IGFBP-3) levels, and GH stimulation tests. Provocative (stimulation) GH testing is indicated for most patients to confirm a diagnosis of GHD. Traditionally, most pediatric endocrinologists defined a "normal" response by a serum GH concentration of >10 ng/ml. After the clinical and biochemical diagnosis of GHD is made, it is typical to obtain an MRI of the brain with narrow cuts through the hypothalamic-pituitary area. The aim of our study is to …

Injured Proximal Tubules Stain Positivity For Arginase-1 To Indicate Acute Tubular Injury (Ati), Jessica Anderson, Neal Blatt, Olabisi Afolayan-Oloye, Hassan Kanaan, Wei Li, Larysa Wickman, Ping Zhang

Conference Presentation Abstracts

Background: A recent study indicates that arginase-1 participates in M2 macrophage-mediated renal tubular regeneration in a rodent model (Shin et al. JASN 2022). As injured proximal tubule cells have a phagocytotic capacity (stain positively for CD68 and kidney injury moleucule-1 in previous studies), we hypothesized that the injured proximal tubules may stain positively for arginase1. Design: We analyzed PAS-stained renal biopsy sections for evidence of ATI using a semiquantitative scale: 0 (no injury) to 1-2+ (mild to moderate ATI) to 3+ (severe ATI). First group (N=7) consisted of renal biopsies that had no evidence of ATI or were obtained from …

Spontaneous Pneumomediastinum In Children Is Not Associated With Esophageal Perforation, Kevin Roby, Catherine Barkach, Diane Studzinsk, Nathan M. Novotny, Begum Akay, Pavan Brahmamdam

Conference Presentation Abstracts

Purpose: We hypothesized that esophageal perforation is not a common etiology for spontaneous pneumomediastinum (SPM), and that SPM is a self-limited disease not requiring routine admission. Methods: We performed a retrospective review of patients, ages 0 to 21 years, diagnosed with SPM in one hospital system (five hospitals) from 2009 – 2019. Cases were identified using ICD9 and 10 codes and confirmed through chart review. Cases were excluded for trauma, severe infections, or subsequent development of SPM after admission for a separate diagnosis. Data regarding patient characteristics, diagnostic tests, and outcomes were collected, and summary statistics were performed. Results: We …

Robotic Spleen Preserving Distal Pancreatectomy (Warshaw Techniqe) In A 12-Year-Old Female., Rachel Harvey, Begum Akay, Pavan Brahmamdam

Conference Presentation Abstracts

Solid pseudopapillary neoplasm (SPN) of the pancreas is a rare entity seen in young females. Complete surgical resection is often curative. This video shows a robotic spleen preserving distal pancreatectomy for a SPN in a 12-year-old female using the Warshaw technique. This technique preserves the short gastric and left gastroepiploic vessels allowing for splenic survival after ligation of the splenic vessels. This video demonstrates the feasibility of using the robot to perform this procedure in a pediatric patient. MRI at 18 months follow up showed that the spleen was completely perfused, with no evidence of recurrence or metastasis.

Perioperative Antibiotic Use In Neonatal General Surgery: A Survey Of Apsa Membership, Ashley Williams, Samuel K. Osei, Pavan Brahmamdam, Begum Akay, Diane Studzinski

Conference Presentation Abstracts

Background: Surgical site infections (SSIs) are a significant source of perioperative morbidity. There is limited data regarding antibiotic prophylaxis in pediatrics, and there are no standardized guidelines in neonates. Thus, peri-operative antibiotic use is based on surgeon experience and preference. To explore this knowledge gap, we sought to describe the practice patterns of pediatric surgeons with regard to antibiotic prophylaxis for SSIs in neonatal surgery

Methods: A survey was distributed to the American Pediatric Surgical Association (APSA). Members were asked to respond to 6 hypothetical case scenarios: Tracheoesophageal fistula/Esophageal atresia, Hirschsprung’s disease, Gastroschisis, Congenital diaphragmatic hernia repair, Neonatal intestinal obstruction …

Cerebral Perfusion Using The Resistive Index On Doppler Ultrasound In Extremely Premature Newborns With Ventriculomegaly, Rawad Obeid, Marni Jacobs, Taeun Chang, An N. Massaro, Dorothy Bulas, Anna Penn

Conference Presentation Abstracts

Background: In extremely premature newborns, the effect of ventriculomegaly, as seen on cranial ultrasound (CUS), on cerebral perfusion is not well studied. The resistive index (RI) (Figure-1) is a marker of change of the flow velocity on Doppler ultrasound that could be used to understand this effect.

Objective: To identify the following: 1- The normal RI values in premature infants without intraventricular hemorrhage (IVH), 2- The relation between the RI in the major intracerebral arteries and the ventricular size on CUS using the fronto-temporal horn ratio (FTHR) in newborns with and without IVH, and 3-The association with white matter injury …
