Nurse burnout is a well-defined problem that has been exacerbated by the COVID-19 pandemic and negatively affects nurses’ mental health, nursing shortages, patient safety, and quality of care. Evidence shows that mindfulness is an effective strategy for managing stress and decreasing burnout in nurses. This article describes an evidence-based practice project that translated this evidence into practice via a vetted mindfulness educational intervention for nurses in intensive care and acute care settings. Background: Nationally, 20% of patient readmissions occur within the first 48 hours after arrival to a skilled nurse facility (SNF), and 40-68% of these readmissions are potentially avoidable. Since 2016, the Centers for Medicare and Medicaid penalized SNFs when their return to hospital (RTH) percentages was above national benchmarks. The four SNFs in this project experienced rising transfer rates and diminished RTH percentages since 2017, despite using in-house providers Monday through Friday. The COVID-19 pandemic brought to light how pure reliance on traditional face-to-face (F2F) learning can be detrimental to student learning. Minimum Data Set (MDS) is used in long-term care facilities to document resident’s information and assessments and to determine the resident’s eligibility to receive Medicare or Medicaid insurance coverage for treatment expenses while in the LTC facility. When reviewing the MDS, the facility identified inaccurate documentation of the resident’s functional mobility ambulating on the unit. This quality improvement project was undertaken to improve assessments, resident interventions, and documentation regarding functional mobility. Purpose/Background Central lines provide lifesaving uses, but they also place the patient at risk for central line associated blood stream infections (CLABSI). Catheter location, length of stay, handwashing, dressing changes, and disinfection of ports before use are all factors to prevent CLABSIs, yet sometimes these measures are not enough. Disinfecting caps are another way to decrease central line infection, however not all hospitals use this method. Critical actions carried out by health care providers to prevent health care errors include accurate patient identification and blood specimen labeling. With a goal of zero harm, improperly identified blood specimens drawn by nurses employed in the acute care setting and Emergency Department at Albert Einstein Medical Center Philadelphia were tabulated and revealed labeling errors. The Nursing Department launched a rapid improvement nurse education plan that resulted in decreased specimen errors compared to baseline counts and suggested the need to sustain error reduction strategies. This Quality Improvement project conducted at a 245-bed acute care hospital in the southeastern United States, aimed to develop an evidence-based preceptor program to improve retention rates. Specific goals were to create a positive experience for preceptors and preceptees by improving the training, support, and recognition. Secondary goal was to increase the number of trained preceptors. The training focused on safe practice, employee satisfaction, and self-efficacy.
