The average literacy rate for healthcare client education materials is currently 11th-16th grade (Dignan & Hunter, 2015). Due to cognitive impairments, difficulty understanding healthcare education materials increases after experiencing a stroke (Danzl et al., 2016). Poor health education results in lower adherence to treatment and limited participation in everyday activities at home (DeMarco, Nystrom, & Salvatore, 2011). Clients and caregivers specifically reported a lack of understanding of post-stroke impairments (O'Connell, Baker, & Prosser, 2003).

Occupational therapy (OT) is an integral component of the rehabilitation process for individuals who have sustained stroke. As the number of individuals sustaining and surviving stroke in the United States has consistently risen over the past decades (Bartels, 2011), it is anticipated that stroke prevalence from 2010 will have a 25% increase by the year 2030 (American Heart Association (AHA), 2011). In addition, a current trend in research has been found, indicating an increase in the incidence of stroke within the young and middle-aged, or younger, adult population between the ages of 18 and 55 years (AHA, 2011).

Based on data from the literature review and clinical experience, an interactive manual was developed for occupational therapists to utilize when working with caregivers of cerebral vascular accident (CVA) survivors. CVA survivors display difficulty in basic personal cares such as eating, dressing, and general mobility within, but not limited to, the first year following the onset of the CVA. An interactive caregiving manual was created that addresses the needs of a caregiver of a CVA survivor during their transition from hospital to home. Several research studies have determined that fatigue is a common symptom, that should be viewed as an independent consequence following a stroke: it interferes with an individual's engagement in activities of daily living. Fatigue can be misinterpreted as depression, therefore, may be treated inappropriately or remain untreated. The literature indicates that approximately 730,000 Americans experience a new or recurrent stroke each year. There are approximately 4 million Americans living with some type of disability as a result of having suffered a stroke. Due to the high incidence of stroke related disability and associated economic costs; stroke rehabilitation is a health care priority. Even with early intervention only 10% of stroke survivors will recover completely, 25% recover with minor impairments, 40% experience moderate to severe impairments, 10% require care in a nursing home, and 15% die shortly after the stroke.
