Ethnographic methods were used to examine the sociolinguistic dimensions of female nurse practitioner interactions with female patients. The purpose was to provide a contextual account of the discursive practices used by the nurse practitioners during routine office visits with female patients. Direction for this research was provided by my concern about power inherent in professional-client dyadic relationships. Data collection methods included formal and informal interviews, participant observation and audio taping nurse-patient encounters. Nine nurse practitioners and 26 patients participated in the study in both private and public ambulatory care settings. Data analysis yielded two concurrent story lines which I labeled …

Few studies have been done that examine the acceptance of nurse practitioners as providers of care by persons infected with the Human Immunodeficiency Virus (HIV). The purpose of this study was to evaluate the level of satisfaction with care as experienced by the HIV-infected client in an outpatient setting who has received care from both nurse practitioners and physicians. The purpose of this study was to identify barriers perceived by homeless families to well child care for their children under age thirteen and to determine if there is a relationship between perceived barriers and duration of homelessness. Using an investigator-modified version of Melnyk's Barriers Scale and a demographic measure, a convenience sample of homeless families (N = 53) from three transitional shelters in two southern California counties were surveyed via questionnaire. Barriers to well child care for homeless children were identified. No relationship was determined to exist between duration of homelessness and perceived barriers using a measurement of correlation.

Hospital quality assurance programs have been provider-oriented, focusing on structure and process elements of care rather than outcomes specific to patient needs and values. This study tested the relationship between quality and value, proposed in Larrabee's model of quality, by developing prediction indices for patient and nurse quality and value.

The sample consisted of 199 adult patients. Patient quality was measured by a 100 mm visual analog scale (VAS) (PQUALG) and a modified version of an existing instrument (PQUALT). The purpose of this study was to determine the level of occupational stress among nurse administrators and to identify the types of strategies used by nurse administrators to deal with or manage occupational stress. The study examined the relationship between selected demographic variables, occupational stress, and strategies. The research design included five research questions along with seven null hypotheses testing the relationship between occupational stress and demographic variables--age, gender, marital status, years of professional nursing experience, years as a nurse administrator, educational attainment, and hospital bed capacity. This descriptive meta-analytic study investigated 73 primary studies on costing out nursing services. A critical review of the literature revealed that findings from the various published and unpublished studies were inconsistent and inconclusive. This meta-analysis integrated the literature to identify the relationships between nursing costs and a second variable. The most frequently reported variables among the primary studies were compared using Pearson r correlations and percentages. The variables of total and direct nursing costs were correlated to the variables of length of stay, direct nursing care hours, hospital costs, and diagnostic related grouping (DRG) reimbursements. Qualitative methodology was employed to conduct a phenomenological inquiry describing the structure of the experience of being pregnant and using drugs. The purpose of the study was to explore the nature of women's experience and perception of the interaction, relationship, and intersection of contextual phenomena of lifestyle, pregnancy, and substance abuse. Data gathering and analysis was accomplished using guidelines provided by Spiegelberg and Van Manen. The purpose of this research was to investigate the effects of an intervention involving employee participation in decision making and behavior modeling training on quality of service. Subjects were nursing aides in two similar nursing home facilities operated by a medium-sized long-term care organization. Participation in decision making involved weekly meetings using a quality-circle-type problem-solving process to develop suggestions for improving quality of service to residents and their families. Behavior modeling training was used to teach interpersonal skills necessary for handling a customer complaint. The purpose of this study is to identify student's and faculty's perceptions of effective characteristics of clinical teachers. The purpose of this study was to determine what the relationship was between levels of trait and state anxiety and performance when giving intramuscular or subcutaneous injections. It was a replication of a study by Megal, Wilken, and Volcek (1987).; In May, 1990, the STAI Form Y-2 was used to assess trait anxiety of 17 students at a community college in the Midwest. State anxiety was measured by using the STAI Form Y-1 immediately before: (1) laboratory performance examinations and (2) giving first injections to clients. The Relationship Between Locus Of Control, Powerlessness, And Job Satisfaction Among Registered Nurses, Evelyn C. Wirsing

A cross-sectional descriptive correlational design was used to investigate the following research questions: (a) How much variation in nurses' satisfaction can be predicted from locus of control and powerlessness? and (b) are there statistically significant (p {dollar}<{dollar}.05) differences in job satisfaction among nurses based upon their primary area of practice, highest earned degree, length of employment in current position, and employment status? A random sample of 300 staff nurses from a large acute care teaching hospital was surveyed utilizing a four-part questionnaire. The response rate was 51% (N = 152). Powerlessness accounted for 29% of the variance in job satisfaction scores and locus of control accounted for none of the variation in job satisfaction. Secondly, a weekly significant difference (F(5, 146) = 2.29, p {dollar}<{dollar}.05) in job satisfaction scores existed among clinical specialty groups, however no significant differences in job satisfaction were found for the remaining professional characteristics.

Post traumatic brain injury symptoms can be very stressful to family caregivers. This study evaluated family caregivers' ability to select appropriate caregiver actions following discharge instructions on post traumatic brain injury symptoms. Subjects in the control group viewed videotaped discharge instructions on physical symptoms post traumatic brain injury. The experimental group viewed videotaped discharge instructions on physical symptoms, cognitive dysfunction and behavioral changes post traumatic brain injury. Both groups were given a written post test after viewing the videotape. Those who received information on physical symptoms, cognitive dysfunction, and behavioral changes had a higher mean score than those who received …

The accuracy of coagulation studies drawn from heparinized arterial lines has been a source of controversy and divergent results of research studies. The discarding of blood to provide an uncontaminated specimen is a concern as a source of infection for health care workers. Hardiness, a personality characteristic that is purposed to mediate the stress response and reduce the likelihood of illness, was studied in the context of adaptation to chronic disease. A descriptive correlational design was used to determine whether a positive relationship exists between the presence of hardiness and psychosocial adaptation to hypertension. The sample consisted of 50 individuals 60 years old or older. Data was analyzed using the product moment correlation coefficient, Spearman's Rho, and multiple regression. A moderately weak correlation (r =.39, df = 48, p =.003) between hardiness and adaptation was found. Other significant relationships identified included those between …

In a prospective experimental study, 24 high risk infants over 1,000 grams and under 5 days of age were observed and evaluated for the effects of positioning on the umbilical artery catheter, activity, and oxygen utilization. Infants were positioned right lateral, left lateral, supine and prone, for 2 hours in each position. The purpose of the study was to evaluate whether prone position opposed to other positions increased sleep state behaviors, reduced oxygen need, or created any complication for the umbilical artery catheter.
