Outcomes Comparison Of Enculturating Advance Directives Process At A Health System, Rose Allen, Tanya Cohn, Christine Edozie, Susan Howard, Patricia Mccrink

The Center for Medicare and Medicaid Services requires organizations to comply with the Patient Self-Determination Act by having processes that inform patients about their rights to execute an advance directive (AD) and engage in shared decision-making. The aim of this study was to compare AD data from a previous study (1999–2002) to a postenculturation (2011–2015) of a structured process for documented patient’s preferences. Second, to conduct a descriptive, bivariate analysis of the enculturated structured ADs process during 2011 and 2015. Reducing Warfarin Adr’S With A Nurse Led Anticoagulation Clinic: A New Model Of Patient Care, Michael Salinero, Tina Hyman

Introduction: Warfarin was the leading cause of drug related adverse events at South Miami Hospital, which resulted in 27 hospital admissions in a year. A retrospective chart review revealed that patients were being discharged without a clear plan for follow up and management. Having optimal outcomes as a result of Warfarin therapy depends on maintaining the INR (International Normalized Ratio) in its range. In order to do so, high-quality anticoagulation management (HQACM) is required.

The assessment of a stroke patient’s ability to swallow plays an important role in the management, recovery and patient outcome in stroke. Nurses in the Neuroscience unit encounter this situation daily in caring for stroke patients. This nursing assessment is integral in providing adequate nursing care to prevent complications such as aspiration pneumonia.
