Abstract

Over the past 40 years there have been significant advances in medical care for HIV. However, many people living with HIV (PLWH) experience internalized stigma in the form of HIV-related shame. This shame has been found to negatively impact mental health, patient relationships, and adherence to healthcare. This grounded theory DNP scholarly project was aimed at developing a novel theoretical framework and practice to improve quality of life including increased viral load suppression in PLWH. Background: The COVID-19 pandemic took the world by storm in the beginning stages of 2020. Public health agencies were overwhelmed, undermanned, and therefore slow to respond. Information began to flow at an unprecedented rate. Yet, very limited resources were put into place to present this information or to instruct the public on evidence-based prevention, treatment, or care at home during active infection.

Abstract Background: With over 130 million emergency room (ER) visits per year in the United States, many emergency departments (ED) are struggling with overcrowded conditions. Contributing to overcrowded conditions are those presenting with dyspnea, who are nearly 4 million in number. ED crowding has serious implications. Exploring and implementing evidence-based methods to alleviate congestion is critical. Problem: One method being explored is the integration of point-of-care ultrasound (POCUS) into the stretcher side assessment of dyspneic patients. Background: Traumatic injury is the leading cause of death for individuals under the age of 45 years old, regardless of race, origin, or sex (CDC, 2019). Among patients who sustain traumatic injury, hemorrhage remains the leading cause of death (American College of Surgeons [ACS], 2014). Hemodynamic stability may be achieved through administration of blood products such as packed red blood cells (PRBC), plasma, and platelets (PTL). Patients with severe exsanguination may require administration of blood products in large quantities, also known as a massive blood transfusion (MBT).

Abstract

Background: Exhaustion is now accepted in nursing culture. Staff suffering from sleepiness and fatigue are more likely to commit medical errors and sentinel events. Emergency department nurses have the greatest risk of committing these errors. Currently, there is no relief provided to shift workers beyond a single 30-minute meal break within a 12-hour period. Other researched mitigation strategies have either failed or are unrealistic in this environment.

Palliative care was not the specialty of many hospitalists; thus, communication was hindered regarding ELC and advanced directives. Despite healthcare utilization in the United States, inadequate treatment of serious illnesses continues due to the lack of provider education and understanding of ELC (palliative and hospice care). Background: Emergency department (ED) overcrowding leads to detrimental results including increased morbidity and mortality, delay in evaluation of treatment, increased cost of care, increased length of stay, diminished staff satisfaction, increased ambulance diversion, and reputation damage for the entire institution (American College of Emergency Physicians, 2019). A study performed at three high volume community hospitals revealed inefficiency of nursing handover as one of the many contributors to prolonged ED length of stay (Salehi, 2019).
