Practice Problem: Chronic obstructive pulmonary disease (COPD) readmissions severely impact patients’ health, mortality, and quality of life and increase unnecessary healthcare use and spending. Practice Problem: Alcohol Use Disorders (AUD) affects a significant portion of the population in the United States.
