Exploring The Influence Of Contextual Factors And The Caregiving Process On Burden, Quality Of Life, And Outcomes Of Heart Failure (Hf) Dyads After A Hospital Discharge Guided By The Individual And Family Self-Management Theory (Ifsmt): A Mixed Method Study, Tamara Bernard
Theses & Dissertations
ABSTRACT The purpose of this study is to explore the influence of contextual factors and caregiving process characteristics on proximal outcomes of the patient and caregiver after discharge from the hospital. The long-term goal of this research is to reduce caregiver burden and improve patient outcomes. Heart failure (HF) is an increasingly common chronic illness with unique caregiving needs and a high rate of hospital readmissions. Caregiver burden has been researched extensively in other areas of medicine such as oncology and dementia and has been reported for stable chronic HF patients in an outpatient setting. However, there is little research …
Frailty Determination And Responsiveness In Left Ventricular Assist Devices, Sarah Schroeder
Frailty Determination And Responsiveness In Left Ventricular Assist Devices, Sarah Schroeder
Theses & Dissertations
Frailty, the state of being vulnerable with susceptibility to decreased age-related physical reserve, is found in 56-79% of adults with end-stage HF receiving durable left ventricular assist device (dLVAD) therapy. Frailty increases risk of adverse events, including increased mortality in the dLVAD population, but less is known about depression, quality of life (QOL), cognition and frailty responsiveness. Therefore, we aimed to describe and compare frailty (5-Meter Walk Test, 5MWT), depression (Patient Health Questionnaire-9, PHQ-9), quality of life (EQ5D Visual Analog Scale, EQ-5D VAS) and cognition (Trailmaking B) from pre- to 12-months post-dLVAD We also compared demographics, clinical characteristics, and post-dLVAD …

An Enhanced Enrollment Process To Impact Appointment Compliance In The Hf-Optimize Clinic, Carolyn Coons, Teri Diederich, Kyana Holder
Doctor of Nursing Practice Projects: College of Nursing
Background The HF-Optimize clinic showed improved utilization of guideline-directed medical therapy (GDMT) along with improved clinical outcomes from 2018 to 2020. Appointment compliance was poor with only 44% of patients completing all HF-Optimize visits. Missed appointments led to worse health outcomes, fragmented care, and provider inefficiencies. Interventions to improve appointment compliance are vital to improving heart failure outcomes. Objective The purpose of this quality improvement project is to evaluate the impact that an enhanced enrollment process has on appointment compliance in a culturally diverse, adult population referred to the HF-Optimize clinic at Nebraska Medicine. Methods This prospective quality improvement project …

Implementing Immersive Virtual Reality Into A Nursing Curriculum, Sue A. Schuelke, Krystal Davis, Sue Barnason
Innovations in Health Sciences Education Journal
Due to workforce demands, new undergraduate nurses are hired directly into fast-paced units and are expected to manage complex patients with rapidly changing conditions and respond to time-sensitive situations. It is important for nurse educators to prepare undergraduate nurses for transition into clinical practice upon graduation. Simulation has been a valuable tool to provide experiential learning and promote clinical decision-making. The next iteration of improving clinical simulation as an experiential learning modality for nursing students is Immersive Virtual Reality (IVR): a realistic, immersive simulation in a 3-dimensional environment that is experienced by body movements and hand controllers. IVR can incorporate …
Ethical Sensitivity Of Padis Care Among Critical Care Nurses: A Mixed Methods Study, Denise Waterfield
Ethical Sensitivity Of Padis Care Among Critical Care Nurses: A Mixed Methods Study, Denise Waterfield
Theses & Dissertations
Critically ill patients often experience distressful and impactful symptoms and conditions that include pain, agitation/sedation, delirium, immobility, and sleep disruption (PADIS). The presence of PADIS can affect recovery and long-term patient outcomes. An integral part of critical care nursing is PADIS prevention, assessment, and management. To better understand the complexities of PADIS care, two reviews of the literature were completed. The first is an integrative review of the use of PADIS assessment tools by critical care nurses. The second is a qualitative systematic review of the literature on the integration of care ethics and nursing workload. Ethical sensitivity of everyday …

Maternal Distress In The Neonatal Intensive Care Unit, Morgan A. Staver
Theses & Dissertations
Maternal distress in the NICU is a common and potentially harmful phenomenon for mothers with a sick infant. The overall purpose of this dissertation was to describe maternal distress and to examine maternal distress among mothers who have infants greater than or equal to 34 weeks of gestation at birth in the NICU. This was accomplished by first conducting a concept analysis of maternal distress in the NICU. Next, an integrative review of the literature was done to assess maternal distress and associated variables in response to infant hospitalization in the NICU. Finally, a mixed methods study was done to …

Evaluation Of Nurse Delirium Screening And Intervention For Hospitalized Older Adult Hip Fracture Patients, Mary Onyarin
Doctor of Nursing Practice Projects: College of Nursing
Introduction: Delirium is associated with longer hospital stays, increased mortality, high medical costs, and poorer functional outcomes (Oh et al., 2014; Robinson & Eiseman, 2008). This project evaluated the use of the Confusion Assessment Method (CAM) tool to screen hospitalized older adult hip fracture patients for delirium. This project also measured the effectiveness of the implementation of the Hospital Elder Life Program (HELP) volunteers as a non-pharmacological intervention to alleviate delirium. This project was implemented on the Orthopedic/Neurological unit of a Midwest tertiary care hospital.
Methods: The study had a retrospective, descriptive design. The investigators evaluated the use of the …
Identifying Moral Distress In Emergency Nurses, Sarah Taylor
Identifying Moral Distress In Emergency Nurses, Sarah Taylor
Doctor of Nursing Practice Projects: College of Nursing
Abstract Moral Distress (MD) is an ongoing reality for emergency department (ED) nurses and often emerges when caring for patients at the end of life. Repeated exposure to MD can result in resignation from the nursing profession. This study measured moral distress using the Measure of Moral Distress for Healthcare Professionals (MMD-HP) survey tool. The goal was to identify stressors known to cause moral distress, measure the frequency of their occurrence during the COVID-19 pandemic, and analyze the correlation to influence ED nurses to leave the position. The MMD-HP ranks the frequency of occurrence of stressful situations leading to moral …
