As part of a nursing leadership project, a team of nursing students at Seattle Pacific University (SPU) collaborated with a Washington State affordable independent living facility for seniors; they focused on providing information on the appropriate use of emergency care, urgent care, and primary care services for various health concerns. This project began with a team member recognizing a pattern within the older adult population entering the Emergency Department (ED). In February of 2023, six Seattle Pacific University (SPU) nursing students partnered with a Seattle rehab agency to deliver mouth care education while prioritizing the health literacy level of the community. The agency aims to foster an inclusive and empathetic environment that builds a diverse community for those traumatized by homelessness, addiction, and mental health (Recovery Café, n.d.). Substance Use Disorder (SUD) is a growing problem in the United States (U.S.) and worldwide and relapse is a common occurrence among those facing SUD. Limited scholarly research has been published addressing evidence-based treatment improvement strategies. This study focused on improving SUD treatment through the incorporation of depression tracking from admission to and discharge from inpatient treatment, as well as the incorporation of social determinants of health education during treatment, with an evaluation of relapse rates 30-days after inpatient treatment discharge. Communities play a role in supporting efforts to improve health literacy. Libraries are trusted institutes that offer an environment for all members of the community and serve as places of lifelong learning. A needs assessment approach determined a gap in knowledge regarding health literacy and insurance, specifically the Affordable Care Act (ACA). The current state of scientific knowledge on using the emergency department (ED) for chronic care management indicates that using the ED for chronic care management creates health disparities and burdens healthcare systems. Ethical concerns also arise because patients use the ED for chronic care management. This article discusses health literacy, self-care behaviors, and social support and the presence of patient suffering, nonmaleficence, and beneficence in patients who seek care for chronic care management in the ED. Focused healthcare provider teaching was used to increase health literacy sensitive care and to improve interdisciplinary collaboration between providers in a mobile clinic setting that serves significantly underserved and socioeconomically challenged populations. PURPOSE: The purpose of this quality improvement project was to assess provider and staff knowledge about health literacy. Specifically, the knowledge of health literacy and perceived importance, current usage, and benefits of the use of a health literacy assessment tool at a primary care clinic was evaluated pre-intervention. The perceived importance of health literacy as well as satisfaction with, frequency of use, and recommendation of the chosen health literacy assessment tool for future use was evaluated post-intervention.
