Chronic Bronchitis is an aspect of the broad disease known as Chronic Obstructive Pulmonary Disease (COPD). It is a widely prevalent disease in the world today and effects 15 million people and is the third leading cause of death. The signs and symptoms of Chronic Bronchitis is very specific, however diagnoses and treatment is different for each patient and can be quite challenging. Chronic obstructive pulmonary disease (COPD) is a chronic inflammatory lung disease that causes obstructed airflow from the lungs. Emphysema and chronic bronchitis are the two most common conditions that contribute to COPD. The purpose of this poster is to investigate and transfer knowledge of the pathophysiological concept of emphysema along with the significance of pathophysiology, and implications of nursing. The research also includes the signs and symptoms and treatment options available. Sources used were published between 2013- 2018 in the United States. The poster provides valuable information for providers to be competent when caring for this patient population.

Despite being a GOLD guideline and having documented benefits, confirming a COPD diagnosis with spirometry is not routinely done. The purpose of this project was to increase patient referrals for spirometry to confirm COPD diagnosis. A quasi-experimental design was incorporated in a primary care office. A retrospective pre-intervention chart audit and two post-chart audits, 30-days apart, compared the frequency of documented spirometry to confirm a COPD diagnosis. An educational intervention with a pre and post-survey examined provider behavior intention to order spirometry to confirm a COPD diagnosis. Purpose. Patients with Chronic Obstructive Pulmonary Disease (COPD) tend to face frequent emergency department visits and hospitalizations. The purpose of this project was to assist a home health agency in the process of building a video visit program to provide additional support to their COPD patient population.

Methods. A literature review was conducted to explore evidence for the use of video visits in home health care. Existing telehealth resources, a chart review, and interviews with stakeholders were utilized to create video visit guidelines. Due to elevated readmission costs, the Affordable Care Act established the Hospital Readmission Reduction Program in 2012 to curb the 30-day readmission rates. COPD and schizophrenia are two very expensive diseases, COPD national medical costs is projected to be $49.0 billion in 2020; the cost of schizophrenia was 155.7 billion in 2013. The main objective of this study was to determine if schizophrenia is a significant predictor of 30-day readmission following hospitalization for acute exacerbation of COPD after adjusting for age, gender, anxiety, smoking status, T2DM, chronic ischemic heart disease, and GERD. Chronic obstructive pulmonary disease (COPD) is a collection of chronic conditions that results in irreparable lung damage and stress to patients. COPD also has considerable financial impacts on health care entities due to frequent hospital readmissions of COPD patients. The Centers for Medicare and Medicaid Services penalize care entities for 30-day hospital readmissions. Many rehospitalizations attributed to COPD are due to exacerbations, often preceded by physiologic and emotional changes that can be monitored, allowing action to be taken to prevent readmissions. The purpose of this evidenced-based project was to answer the question: Does using a formalized discharge list identifying key educational topics regarding the patient’s disease process decrease readmission risk for COPD patients? There was no standardized education offered to the COPD population at the practice location. Many patients are affected by chronic obstructive pulmonary disease (COPD), a progressive lung disease that obstructs air flow, resulting in dyspnea and inability to carry out daily activities.
