The purpose of this project was to implement an APN-led COPD discharge education program to decrease 30-day readmission rates. This Doctorate of Nursing (DNP) project combined strategies obtained in the literature search and blended these into a cutting-edge and state-of-the-art discharge education program at a major medical center. The purpose of this study was to determine the effect of participation in a pulmonary rehabilitation (PR) program on depressions scores in persons with COPD and if there is a difference between males and females in depression scores prior to and following participation in a pulmonary rehabilitation program. The literature review supported multiple factors on how participating in a pulmonary rehabilitation program can have a positive effect on depression scores. Chronic obstructive pulmonary disease (COPD) is the number one disease process treated in the Pulmonary Department at the Chalmers P. Wylie VA Ambulatory Care Center (VAACC). The medical staff includes doctors, nurses, and respiratory therapists. The team works together to ensure the veteran gets the best care available. Smoking is very popular in the military, and this puts veterans at a higher risk for COPD, compared to the general public. COPD can develop at a much younger age if the patient has alpha-1 antitrypsin deficiency (AATD).
