Cross-Cultural Reliability Of The Health Control & Competence Index And The Health Perception Index, Dr. Vincent L. Salyers
Cross-Cultural Reliability Of The Health Control & Competence Index And The Health Perception Index, Dr. Vincent L. Salyers

It is well documented that men of Latino origin are subject to health disparities with an increased morbidity and mortality from preventable diseases. Despite their proclivity for health problems, these men also have the lowest participation in health promotion services. This may be due to health disparity factors, health access factors, cultural factors, gender role factors, and constraints to receiving culturally competent health care. Addressing such factors as health knowledge, health perceptions, access to care, and culturally/linguistically competent care is essential to increasing participation in health care, health promotion and screening activities by men of Latino origin. This mixed-measures exploratory …

Behavioural Effects Of Nicotine Exposure From Secondhand Smoke Among Bar And Restaurant Workers, Chizimuzo T.C. Okoli, Ellen J. Hahn, Mary Kay Rayens
Chizimuzo T.C. Okoli
Introduction
• Besides the adverse physical health effects of secondhand tobacco smoke (SHS) exposure, not much is known about its behavioural outcomes.
• SHS exposure is a source of involuntary nicotine exposure with potential behavioural consequences in smokers and nonsmokers alike.
• Hospitality workers are a population at high risk for health problems related to occupational SHS exposure.
• This study explores self-reported behavioural symptoms commonly associated with nicotine withdrawal among smoking and nonsmoking bar and restaurant workers exposed to SHS.
Methods • This is a cross-sectional analysis of existing data obtained from a larger study examining the effect of …

A Critical Case Study Of Program Fidelity In Tenncare, Carole R. Myers Dr.
A Critical Case Study Of Program Fidelity In Tenncare, Carole R. Myers Dr.
Carole R. Myers, PhD, RN Associate Professor
Purpose: The purpose of this study was to evaluate the fidelity of the design of Tennessee’s Medicaid managed care program in comparison to the actual program operation. Program fidelity is a broad measurement of how true the implemented program is to the intended program (Heflinger & Northrup, 2000). Background: In the span of only 15 years, the introduction of managed care and other market-based strategies from the private sector precipitated a transformation of the delivery of Medicaid services in the United States. These monumental changes remain poorly understood. The implementation of managed care in Tennessee’s Medicaid program is an excellent …
An Exploration Of The Meanings Of Space And Place In Acute Psychiatric Care, Melanie Andes, Mona Shattell
An Exploration Of The Meanings Of Space And Place In Acute Psychiatric Care, Melanie Andes, Mona Shattell

Spatial human experiences such as confinement and freedom are important to acute psychiatric care. The physical space that inpatient psychiatric/mental health nurses and acute psychiatric patients share influences human relationships. The purpose of this paper is to explore the meanings of space and place in acute psychiatric settings, to discuss how these meanings affect human relationships, nurses’ work environment and patients’ perception of care, and to present how the design and use of nursing stations affects therapeutic relationships. We hope to encourage dialogue and research that will help clarify the meaning of space and place in acute care psychiatric units, …

Promoting Local Implementation Through Community Cancer Research Review Work Groups In Rural Appalachia, Bruce Behringer, Karen Harrell Mabe, Sadie P. Hutson
Dr. Sadie P. Hutson, PhD, RN
Underserved communities with high cancer rates often are not involved in implementing state cancer control activities locally. An East Tennessee State University research team formed 2 Appalachian Community Cancer Research Review Work Groups, 1 in northeast Tennessee and 1 in southwest Virginia. During 4 sessions, the research team presented regional cancer data to the work groups. Work group participants explored research from a lay perspective and identified possible reasons for cancer disparities in central Appalachia. The fifth session was a community dissemination activity in which work group participants engaged in cancer education and action by presenting the research to their …

The Chronic Illness Quality of Life Ladder (CIQOLL) underwent psychometric testing in a sample of 278 women with HIV disease. The CIQOLL, a self-anchoring striving scale based on Cantril’s Ladder, measures seven domains (physical , emotional, financial, family and friends, spiritual well-being, peace of mind, and overall life satisfaction) across four time periods (present, past, future, life without a diagnosis of HIV). The domains were derived from focus groups with persons with HIV disease. Women with a diagnosis of HIV Infection, age 18 or older, residing in rural areas in the southeastern United States, completed questionnaires that measured physical functioning, …

The purpose of this study was to evaluate the feasibility of measuring stress hormones and immunity following rape. The long-term goal is to evaluate the predictive value of stress-immune-inflammatory responses to later health outcomes. Fifteen women reporting rape were compared with 16 control participants. Serum stress hormones, proinflammatory cytokines, acute phase proteins, functional assays, and lymphocyte subsets were measured in blood samples. Women reporting rape had higher cytotoxic cells, lower B lymphocyte counts, higher proinflammatory biomarkers, and decreased lymphocyte proliferation compared to the control group. This finding suggests that rape produces activation of the innate immunity and suppression of some …
Talking It Over With The Gals: An Analysis Of Editorials By Phyllis Stern, Sandra Thomas
Talking It Over With The Gals: An Analysis Of Editorials By Phyllis Stern, Sandra Thomas
Sandra Thomas
Editorials written by a journal editor proved a glimpse of that individual's philosophy and opinions about topical events. The purpose of the present project was to analyze the editorials of Phyllis Noerager Stern during the years that she served as editor-in-chief of Health Care for Women International (1983-2001). Using ethnographic content analysis, 65 editorials were examined to ascertain: (1) what event triggered the editorial; (2) what aim or objective could be identified; and (3) what rhetorical devices the editor employed. Personal narratives and editorials about the abuse/exploitation of women made up the largest categories. Stern's grounded theory orientation, fascination with …

Purpose: To explore the experience of registered nurses (RNs) caring for patients in contemporary hospitals.
Design: The descriptive phenomenological study was based in the philosophical perspectives of Husserl and Merleau-Ponty.
Methods: A purposive sample of 46 RNs employed in acute care hospitals in the southeastern United States (US) were recruited by network sampling. Data from unstructured interviews were analyzed in an interpretive group and themes were identified.
Findings: Four themes were identified: (a) extraordinary caregiving events, (b) incomprehensibility, (c) questioning whether anything else could have been done, and (d) “alone or together,” indicating the isolation nurses often experience while giving …

The foundation of psychiatric/mental health nursing is the relationship between nurse and patient. Caring for persons with mental illness within the context of this relationship requires knowledge of the individual. To gain this knowledge, understanding the patient’s perceptions and concerns is essential. Research suggests that this understanding does not always occur. The study reported here examined what it means to individuals with mental illness to be understood. In-depth phenomenological interviews revealed three predominant themes: “I was important,” “It really made us connect,” and “They got on my level.” Nurses can use these findings to improve the care of those with …
