Patients in the intensive care unit (ICU) are critically ill and faced with numerous physical and psychological stressors that often require a multidisciplinary approach to improve outcomes and relieve suffering. Patients are given sedation to tolerate invasive treatments and procedures. Using the Richmond-Agitation Sedation Score (RASS), nurses assess objective findings and the level of sedation a patient is experiencing, and titrate sedation according to an ordered RASS score. Ineffective and inaccurate use of the RASS score leads to patients being oversedated and demonstrates a lack of understanding of how to appropriately use the RASS score assessment tool. Despite extensive research revealing the dangers of physical restraint use, it remains a common practice in critical care settings across the globe. Nurses stand at the forefront of clinical decision-making regarding the use of physical restraints but are doing so with a lack of education and evidence-based practices. The importance of nurse residency programs is addressed in the literature; however, a review of residency program outcomes and effectiveness is needed. Guided by Roy's adaptation model and Deming's plan-do-check-act model, the purpose of this quality improvement project was to assess the current state of a longstanding critical care nurse residency program in meeting organizational goals and objectives and to recommend modifications to the program related to external factors, internal challenges, and educational deficits of nurses entering the program. Malnutrition among hospitalized patients is prevalent and associated with adverse outcomes. At the health care facility for which this quality improvement (QI) initiative was developed, patients were not consistently fed within the nationally recommended 48 hours. The purpose of this project was to facilitate the early initiation of enteral feedings to prevent malnutrition in a vulnerable patient group by development of an evidence-based enteral feeding policy, algorithm, and nursing education module.
