Utilization Of Care By Infants With Neonatal Abstinence Syndrome In Delaware, Michele K. Savin, Msn, Nnp-Bc

From 1999-2013, the nationwide incidence of neonatal abstinence syndrome (NAS) due to maternal drug use rose from 1.5/1000 to 6/1000 hospital births. There is a dearth of information regarding what happens to these children when they are discharged. The purpose of this Practice Inquiry Project is to utilize existing Delaware Medicaid data to retrospectively explore the utilization of services and gaps in care for the infant with NAS in the first year of life. I am writing in response to “Pharmacologic Treatment of Opioid Addiction During Pregnancy” by Keough and Fantasia (2017). The article nicely compares and summarizes the use of methadone and buprenorphine to treat substance use in pregnant women. I was dismayed, however, by the sections regarding neonatal considerations and breastfeeding, which contained some problematic assertions and some inaccurate conclusions.

Drug use is on the rise in Delaware, as demonstrated by the continued increase in infants born with neonatal abstinence syndrome. Thoughful, evidence based, and coordinated approaches are necessary to impact this problem. There is solid evidence that mothers and infants who remain together have improved outcomes. Professional medical and nursing societies are unanimous in support of non-punitive approaches to care. Medical Professionals, legislators, and society in general would benefit from ongoing education on the addiction disease process in order to best care for the increasing number of mother/baby dyads with neonatal abstinence syndrome.

BACKGROUND: Since their creation more than 20 years ago, the proliferation of Quick Response (QR) codes has expanded tremendously. Little was found in the literature to support the innovative use of QR coding in the classroom or in health care provision. Thus, the authors created a doctoral-level practicum experience using QR coding to create interactive, individualized patient or provider resource guides.

METHOD: Short, descriptive surveys were used before and after implementation of the practicum experience to determine students' comfort level using QR technology, their knowledge base, ease of use, and overall satisfaction with the practicum.

With the introduction of tablet technology, faculty began asking questions about when, where, and how tables might enhance curriculum opportunities for student learning and faculty teaching strategies beyond their use as personal digital devices and smartphones (Swan, Smith, Frisby, Shaffer, & Hason-Zalot). Personal digital devices (PDDs), including smartphones, had been used by nursing students for the past five years. The purpose of this study was to evaluate how tablet technology affected student and faculty teaching-learning processes and outcomes in an accelerated, pre-licensure baccalaureate nursing program.

In 2011, my husband, Eric, a trial attorney, was felled by a brain stem stroke just before he was to board a flight at O'Hare in Chicago. He was just 53 years old with no prior health conditions or problems. From the outset, we knew his recovery and rehabilitation would be long and difficult. Background/ Rationale: It has been established that Interprofessional Education (IPE) offers a solution to foster teamwork and improve patient care and should be implemented early in professional role development. Thomas Jefferson University has been providing IPE to interprofessional healthcare students for over 5 years. A long term strategic goal was identified to “create innovative learning environments that support interprofessional education including state-of-the-art technologies”. Given the ample evidence delirium in hospitalized older adults is under diagnosed and not recognized by nurses up to 85% of the time, interventions are needed at the point of care to assist nurses in recognizing delirium. In this qualitative study, factors that might influence the implementation of a point-of-care screening tool for delirium were examined.
