Chronic Disease Self-Management Support In Rural Areas: An Integrative Review, Karla A. Callahan Jul 2021

Chronic Disease Self-Management Support In Rural Areas: An Integrative Review, Karla A. Callahan

Doctoral Dissertations and Projects

Millions of Americans live with chronic health conditions which require careful attention and specific actions for optimal management. Chronic Disease Self-Management Education (CDSME) has been effectively utilized to improve health outcomes and quality of life for individuals with chronic disease, enhancing knowledge and self-efficacy. Traditional CDSME programs are less available for rural residents. An examination of successful CDSME programs highlights common interventions that yield success and may be utilized in a variety of settings. Self-management is more complex, but also more essential, for individuals with more than one chronic condition. Clinicians can provide individualized support for appropriate self-management techniques, thereby …

Type 2 diabetes mellitus is a significant health issue, as it requires patients to perform daily self-management activities. One in every three Americans with Type 2 diabetes has limited or low health literacy skills. Limited or low health literacy skills can impact a patient’s overall health outcome; therefore, a critical element for disease management and patient adherence is health literacy. The Incorporation Of Evidence-Based Simulated Activities In An Existing Nurse Residency Program, Wendy Partin Thompson

This problem is especially evident in nurses with less than one year of clinical experience. Research has shown that nurse residency programs without simulative activities eliminate vital critical thinking activities which prepare novice nurses for clinical practice. The purpose of this project was to design evidence-based simulative activities to increase …

Educational Intervention To Improve Self-Efficacy And Self-Care In Patients With Heart Failure, Helen Parke Aug 2017

Educational Intervention To Improve Self-Efficacy And Self-Care In Patients With Heart Failure, Helen Parke

Doctoral Dissertations and Scholarly Projects

Heart failure is a high mortality, chronic disease that is economically and physiologically costly for patients, caregivers, and society. Heart failure teaching alone is insufficient in meeting the complex self-care needs of the heart failure patient. This project implemented evidence-based teaching methods that are skill-based and problem-solving based to educate heart failure patients in an outpatient setting. The theoretical foundation of this project was Bandura’s theory of self-efficacy and Orem’s theory of self-care. The level of self-care and self-efficacy was assessed before and after the multi-sensory teaching intervention. Self-care and self-efficacy have been studied and shown to be major components …
