What Do Extension And Public Health Educators Need To Address Substance Misuse?, Lisa Washburn, Karen Franck

Limited organizational capacity has hindered Extension’s ability to address substance misuse in communities. To inform capacity-building efforts, we obtained Extension educator and health educator feedback on opportunities, challenges and resource needs using the Delphi technique. Opportunities included collaboration and partnerships with local entities. Stigma, inadequate resources, and limited knowledge and skills of educators were challenges. Top resource needs were updated youth curricula, funding, and clear guidance on what educators can do and what should be referred externally. Findings provide insights to educator perceptions and suggest additional training and resources needed to implement evidence-based approaches to address substance misuse.

The opioid epidemic in West Virginia is a complex interplay of factors, including poverty, low education, and high unemployment rates, which contribute to higher rates of substance use and opioid-related deaths. The number of people injecting drugs has risen from 36% in 2005 to 54% in 2015. WV recorded 871 overdose deaths from all drugs in 2019 with 76.8% involving at least one opioid. Objective
We aimed to assess physicians' perceptions of barriers to starting medication-assisted treatment (MAT) in the Emergency Department (ED), views of the utility of MAT, and abilities to link patients with opioid use disorder (OUD) to MAT programs in their respective communities.
Methods
This was a cross-sectional survey study of American emergency medicine (EM) physicians with a self-administered online survey via SurveyMonkey (Survey Monkey, San Mateo, California). The survey was emailed to the Council of Residency Directors in Emergency Medicine (CORD) listserv and HCA Healthcare affiliated EM residency programs' listservs. Attendings and residents of all post-graduate years participated. Patient and provider knowledge of safe pharmaceutical disposal procedures is crucial from both a public health and environmental standpoint. Unfortunately, studies have shown that Vermont residents are largely unaware of safe drug disposal programs in their communities. This project evaluated patient knowledge of drug disposal practices and awareness of safe pharmaceutical disposal procedures at Hinesburg Family Medicine. A short phrase was created to better disseminate information about safe drug disposal in Hinesburg, which can be added to a patient’s after visit summary. For the past two decades the United States has been embroiled in a prescription drug epidemic that has since grown in magnitude and complexity. The ripples of this epidemic have been especially apparent in the state of Maine, which has fought hard to mitigate the damage caused by addiction to pharmaceutical and illicit opioids. Us- ing data from state and federal sources, we construct a mathematical model capturing the dynamics of the opioid epidemic in the state of Maine, specifically as it pertains to pharmaceutical opioids and heroin. The opioid crisis in the United States has grown at an alarming rate. Children with cancer are at high risk for pain, and opioids are a first‐line treatment in this population. Accordingly, there is an urgent need to optimize pain management in children with cancer without contributing to the opioid crisis. This report details opportunities for this optimization, including clinical practice guidelines, comprehensive approaches to pain management, mobile health, and telemedicine. It is vital to balance appropriate use of analgesics with efforts to prevent misuse in order to reduce unnecessary suffering and minimize unintended harms.

For the past fifty years, American drug policy has been manipulated and enforced in a way that made it possible for drug epidemics to occur and has exaggerated their negative consequences on society. The War on Drugs policy initiatives first implemented in the 1970s created a drug law enforcement structure that has criminalized addiction and made it difficult for addicts to receive treatment. The United States is currently facing it's worst drug epidemic in history due to these policies.
