A Study Of The Clinical Features Of Renal Papillary Necrosis, Jose Martinez

Renal papillary necrosis is a disease entity that has been recognized since 1878. Today, one hundred years later, a considerable amount of knowledge has been gathered about its possible etiologies, associated conditions, pathological pic ture, clinical manifestations, radiological features and mode of treatment. Very little is known, however, about prognosis of this disease. The literature is deficient of prognostic studies that may help the clinician predict the course of patients with renal papillary necrosis. This study is aimed to provide some answers to questions in the area of natural history and clinical features of this disease.

Methods and Materials: Twelve children with CIBD (7 with CUC and 5 with Crohn's) entered a trial with Cromolyn Sodium. The patient groups consisted mostly of those who had been treated with steroids over a 1-year period without suppression of disease activity, and in whom steroids could not be tapered without exacerbation. On commencement of Cromolyn therapy, steroid dosages ranged from 5-60 mg. q.d. (mean dose 30 mg. q.d.). Multiple complications of steroid therapy were observed in this group. The development of the aortic isthmus, that region of the arch of the aorta between the left subclavian artery and the attachment of the ligamentum arteriosum, has received little attention. While the developmental history of the ductus arteriosus and foramen ovale has been well documented, as have the cardiac malformations associated with failure to complete this developmental sequence, little work has been done to define the normal sequence of isthmus development in its transition to an adult configuration in the postnatal period.
