With the complexities that surround myocardial ischemia/reperfusion (MI/R) injury, therapies adjunctive to reperfusion that elicit beneficial pleiotropic effects and do not overlap with standard of care are necessary. This study found that the mitochondrial-derived peptide S14G-humanin (HNG) (2 mg/kg), an analogue of humanin, reduced infarct size in a large animal model of MI/R. However, when ischemic time was increased, the infarct-sparing effects were abolished with the same dose of HNG. Thus, although the 60-min MI/R study showed that HNG cardioprotection translates beyond small animal models, further studies are needed to optimize HNG therapy for longer, more patient-relevant periods of cardiac …

Acute myocardial infarction is primarily due to coronary atherosclerotic plaque rupture and subsequent thrombus formation. Platelets play a key role in the genesis and progression of both atherosclerosis and thrombosis. Since platelets are anuclear cells that inherit their mRNA from megakaryocyte precursors and maintain it unchanged during their life span, gene expression profiling at the time of an acute myocardial infarction provides information concerning the platelet gene expression preceding the coronary event. In ST-segment elevation myocardial infarction (STEMI), a gene-by-gene analysis of the platelet gene expression identified five differentially expressed genes: FKBP5, S100P, SAMSN1, CLEC4E and S100A12. Aim: Homocysteine is known to inhibit endothelial cell proliferation, which is a key event in angiogenesis. Factors responsible for the presence or absence of coronary collateral circulation are poorly understood. Therefore, in this study we investigated the effect of plasma homocysteine level on the early formation of angiographically visible collaterals after acute myocardial infarction. Materials and Methods: The study included 60 patients that had ST-segment elevation myocardial infarction (STEMI). Aim: To investigate the clinical implications of serum heart-type fatty acid-binding protein (H-FABP) compared to myoglobin, cardiac troponin I (cTnI), and creatine kinase MB isoenzyme (CK-MB) in patients with early phase acute myocardial infarction (AMI). Material and Methods: Patients were grouped clinically according to the American College of Cardiology/European Society of Cardiology new definition of myocardial infarction (MI) and by clinician diagnosis of MI. Antiplatelet therapy reduces the risk of subsequent ischemic events in patients suffering from acute myocardial infarction (AMI). In this respect, the multi-agent therapy to be used along with aspirin remains to be determined. In this study, ticlopidine was tried in AMI for its potential additive effects over aspirin. Ninety patients with similar clinical characteristics were involved and were followed up for 3 and 6 months. All patients were assigned to receive ticlopidine plus aspirin or aspirin alone. Cardiac Troponin T (TnT) is a regulatory contractile protein not normally found in blood. Its detection in the circulation has been shown to be a sensitive and specific marker for myocardial cell damage. This study, designed to investigate the blood lactate and pyruvate levels in AMI, was carried out on 41 patients. Based upon the localization of AMI, the patients were studied under anterior AMI (n=17), anteroseptal AMI (n=9) and inferior AMI (n=15) groups. Left ventricular failure developed in five patients with anterior AMI and in one patient with anteroseptal AMI. Cardiogenic shock developed in two patients with anterior AMI.
