A female, age 78, with generalized anxiety disorder (GAD), history of major depression, and hypothyroidism presents for mood and anxiety concerns. Her mood is depressed, and sleep, appetite, energy, and concentration are poor. She recently saw a geriatric psychiatrist and described no past manic episodes, psychotic symptoms, or family history for bipolar disorder. No one has concerns about her cognition. She began levothyroxine 150 mcg 3 weeks ago for newly diagnosed hypothyroidism. On exam, she is talkative, overinclusive, has almost pressured speech, labile mood, and lid lag. Neuropsychology testing, CBC, and CMP are normal. TSH is 0.173. She is diagnosed …

Interrogating Race And Place-Based Inequities In Hiv And Covid-19, Rohan Khazanchi

EMET Projects

Over the last four years, I have developed a research focus examining the intersections of race, place, and health. My M.D. Honors Thesis reflects a snapshot of these efforts. In this collection of brief research reports, I leverage area-based measures to investigate structural inequities in three contexts: the HIV epidemic in our hyperlocal community, the early stages of the COVID-19 pandemic, and clinical trials for novel COVID-19 therapeutics. I apply novel social epidemiologic tools to measure and explore disparate outcomes. And, in reflecting upon my findings, I discuss concrete implications for clinicians, researchers, and policymakers alike.

Chapter 1: Neighborhood-Level Deprivation …

Reducing "Treble" With Performance Focused Music Programs In Medical School: A Student Driven Needs Assessment To Clarify Participation Barriers Amongst Undergraduate Medical Students, Alexander Tu, Tiffany Truong, Kristy J. Carlson Phd, Matthew J. Brooks Dma, Jayme R. Dowdall Md

EMET Projects

Integration of the arts into medical education has numerous benefits, including reducing burnout and producing empathetic physicians. Investment in arts-related activities has expanded considerably. However, funding remains a common limitation for further integration of arts into medical education. Clarifying student preferences and barriers to music programs can optimize such limited resources to best suit medical student needs.

Introduction: It is critical to evaluate student experience with any newly integrated educational resource. In 2018, a Distal Upper Limb (DUL) Anatomy eModule was developed for first-year medical students at the University of Nebraska Medical Center, who have historically identified the DUL as a region of difficulty. This mixed methods study sought to (1) evaluate learner perception of the eModule relative to other resources, and (2) identify eModule content and features that students found valuable.

Methods: The DUL eModule was made available to first-year medical students in 2019 (n= 132), 2020 (n=131), and 2021 (n=131) as a voluntary, …

Do I Add Value? A Mixed Methods Study Comparing Medical Students’ Experiences With Patient Documentation, Shannon Haines, Kari Nelson, Kaeli Samson, Nathan Gollehon

Child Health Research Institute Pediatric Research Forum

Background

Clinical documentation is an essential skill for practicing physicians, yet many medical students are not prepared for the documentation demands required during residency.1,2 Recent CMS rule changes now allow medical students’ notes to be used for billing.3 Previous studies have shown positive perceptions of medical students whose notes were used in an outpatient setting.4 We utilized Constructivist Learning Theory to frame our work, where students construct their knowledge as faculty and residents facilitate the process.5

Objectives

To explore if using student documentation for billing impacts student perceptions and experiences during the pediatric clerkship.

Background

Clinical documentation is an essential skill for practicing physicians, yet many medical students are not prepared for documentation required during residency.1,2 CMS rule changes now allow medical students’ notes to be used for billing.3 Residents are often the primary authors of notes and provide medical student supervision and instruction. However, little is known about how the use of medical student notes in patient documentation would impact residents. We utilized Constructivist Learning Theory to frame our work, where students construct their knowledge as residents and subsequently faculty facilitate the process.4

Malondialdehyde Acetaldehyde Adducts (Maa-Adducts) Direct Distinctive Pro-Inflammatory Responses In Endothelial And Macrophage Cell Lines, Patrick J. Opperman, Michael J. Duryee, Ted R. Mikuls, Geoffrey M. Thiele, Dahn L. Clemens, Daniel R. Anderson

EMET Projects

Chronic inflammation plays a critical role in the pathogenesis of atherosclerosis. At present, the mechanism(s) by which inflammation contributes to this disease isnot entirely understood. Inflammation is known to induce oxidative stress, of which one consequence is lipid peroxidation. This process leads to the production of malondialdehyde (MDA), which can subsequently break down to form acetaldehyde (AA). These two aldehyde by-products can covalently interact with the ε-amino group of lysineswithin proteins and lipoproteins leading to the formation of highly immunogenic malondialdehyde-acetaldehyde adducts (MAA-adducts). The aim of this study was to determine the in-vitro cytokine response of endothelial cells and macrophages …

Thiele, Katherine Janike, Amy J. Nelson, Michael J. Duryee, Bryant R. England, Debra J. Romberger, Joseph Carrington, Dong Wang, Benjamin J. Swanson, Lynell W. Klassen, Ted R. Mikuls, Jill A. Poole

EMET Projects

Asthma, chronic bronchitis and COPD are common adverse respiratory health effects among persons exposed to agriculture organic dust work environments.

  • Occupational inhalant exposures have been increasingly associated with the risk of rheumatoid arthritis (RA) disease development, particularly among males. Agriculture workers have increased risk of RA and generalized bone disease.
  • Chronic lung disease is associated with production of characteristic autoantibodies associated with RA (e.g.anti-citrullinated antibodies), even in absence of RA disease.
  • The mechanistic link between pulmonary inflammation and arthritis (and vice versa) remains poorly understood. Animal models are lacking.

Parkinson's disease (PD) is a neurodegenerative movement disorder in which symptoms derive from deficits in dopamine neurotransmitter levels secondary to loss of dopaminergic neurons in the substantia nigra pars compacta (SNpc) associated with misfolding and accumulation of α−synuclein. Neuroinflammation via microglia and T effector cells (Teffs) contribute to dopaminergic neuronal cell death. Recognition of cytokine profiles of pro-inflammatory microglia is not well understood and serve as potential therapeutic targets to reduce neuroinflammation. Recent studies demonstrated a novel Th17.1 Teff clonotype increases neurotoxicity. The aim of this study was to demonstrate in vitro cytokine responses by BV-2 microglia induced by Th1, …
