A pamphlet consolidating the most used and current resources for housing and food insecurity in Burlington Vermont in the spring of 2024. I collected the list from social workers at my clinic, as well as several unhoused members of the Burlington community. I also corresponded with a provider at the Community Health Center of Burlington who said that information on safe sharps disposal would be useful, so I created instructions on how to safely dispose sharps on the street.

Many people in Androscoggin county are not able to consistently achieve a diet with the recommended food variety for improving health. Adequate nutrition for improving health outcomes requires access to healthy food at affordable prices, health literacy regarding diet and nutrition concepts, and the skills and access to equipment required to prepare and store food. A lack of resources in any one of these areas can limit a person’s ability to engage in a healthy diet, which increases their risk of developing preventable non-communicable diseases later in life.

Food insecurity rates are high among older adults who have chronic conditions and/or are homebound. These patients have functional limitations, comorbidities limiting their mobility, and often are socially isolated or live alone, all of which impact their ability to access or make healthy meals. The COVID-19 pandemic has affected aspects of health and community beyond the acute phase of COVID-19 infection, including chronic health conditions, mental health, substance use, food insecurity, access to healthcare, and housing insecurity. Further, existing disparities in these factors and other social determinants of health such as socioeconomic status, education, neighborhood, and physical environment have been exacerbated by the response to the COVID- 19 pandemic. Community members face negative health impacts as a result of the COVID-19 pandemic and response. They should be made aware of community resources for immediate and long-term solutions to the difficulties they experience.

Food insecurity is defined as a lack of consistent access to enough food for an active, healthy life. Litchfield county, home to New Milford, CT, suffered severe economic changes over the Covid-19 pandemic. The number of food insecure households, represented by those requesting food from local food pantries, skyrocketed. This study demonstrates the importance of screening for food insecurity in primary care. A 3-part screen is implemented in a primary care setting to better assess those at risk and connect them to resources and local food services.

The ongoing COVID-19 pandemic has had a tremendous impact on many individual's lives and their families. It has caused many people to change their lifestyles and daily routines. This project focuses on how diet has changed during the COVID pandemic and explores the reasons for these diet changes. 23 patients from the Central Vermont Medical Center were given surveys and their responses were analyzed and recorded. Recent years have shown an emergence of interest in social determinants of health by healthcare providers, community leaders, and social service organizations alike. Among the social determinants of health, food insecurity has been associated with higher incidences of chronic disease and poor health outcomes as compared to rates among individuals not screening positive for food insecurity. This project aims to compare the effectiveness of screening patients for food insecurity via formal paper questionnaires versus the traditional approach of an open patient-provider dialog guided by provider intuition in identifying food insecure individuals in a primary care setting.

Food Insecurity and access to healthy food is a large health care issue in the United States, Vermont, and Chittenden County. This project aimed to educate health care providers at Colchester Family Practice about food insecurity to increase the amount of patients screened for food insecurity. The project also investigated local resources for people with food insecurity and made this information available to providers so they are better able to help people struggling with food insecurity.

Food insecurity screening is needed to link food insecure patients to interventions that will help improve their nutrition and overall health such as annual community supported agriculture (CSA) shares. In Vermont, food insecurity is widespread, as 76% of Vermonters experience some level of food insecurity. This has serious health consequences. Food insecure patients are more likely to suffer from hypertension and diabetes than non-food insecure patients. Clinicians need to screen for and detect food insecurity. Based on metrics including poor health, hospitalizations, and lower educational productivity, food insecurity cost the State of Vermont $340,000,000 in 2010 alone.
