Background: Dementia is a public health problem that is estimated to triple by 2050.Globally, dementia is the seventh leading cause of death among all diseases and one of the major causes of disability and dependency among older people. The direct medical costs and indirect social and economic costs of dementia are burdensome to society, and by 2030 it is projected to cost more than $2.8 TRILLION dollars worldwide. As the population ages, the prevalence rates of dementia continue to increase. Without a cure or promising treatment for dementia, the best course of lowering the prevalence rates of dementia is through preventative measures. The people of Appalachia have less access to healthcare resources subsequently leading to deficits in healthcare, health education, and health research. There is a need to address the communication of healthcare resources among populations with low economic status such as the Appalachian communities in Northeast Tennessee. Alzheimer’s disease and related dementias (ADRD) uniquely challenges the healthcare industry while further dividing communities already experiencing hardship in healthcare resources. Aims: We sought to identify differences in pain management between two groups; nursing home residents with malignant cancer and dementia with and without hospice services.

Methods: Decedent records from 2003-2009 were assessed for diagnosis of dementia and cause of death as cancer. Ten malignant cancer diagnoses were determined a priori from the CDC 2004 data on the top 10 malignant cancers for all races and genders. Fifty-five decedents from 10 nursing homes were included in the final sample. Four instruments were used: Minimum Data Set (MDS) a standardized assessment tool required of most U.S. nursing homes.
